Overview of the medicine
Olmezest-H 20 mg tablet is a combination medicine used to treat high blood pressure (hypertension). It contains Olmesartan (an Angiotensin II Receptor Blocker) and Hydrochlorothiazide (a thiazide diuretic), which work together to effectively lower blood pressure.
Uses & Indications
Dosage
Adults
The usual recommended dose is one tablet (Olmesartan 20 mg/Hydrochlorothiazide 12.5 mg) once daily. Dose may be adjusted after 2-4 weeks if needed, up to a maximum of Olmesartan 40 mg/Hydrochlorothiazide 25 mg daily.
Elderly
No specific dose adjustment is generally required, but caution should be exercised, especially in patients with impaired renal function.
Renal_impairment
Dose adjustment is necessary based on creatinine clearance. Contraindicated in patients with severe renal impairment (creatinine clearance < 30 mL/min).
How to Take
Take the tablet orally, with or without food, preferably at the same time each day to maintain consistent blood levels. Swallow the tablet whole with water.
Mechanism of Action
Olmesartan blocks the action of angiotensin II, a natural substance in the body that constricts blood vessels, thereby relaxing blood vessels and lowering blood pressure. Hydrochlorothiazide is a diuretic that increases the excretion of sodium and water from the body, leading to a reduction in blood volume and blood pressure.

Olmesartan: Primarily excreted via bile (major) and urine (minor). Hydrochlorothiazide: Excreted primarily unchanged in urine.
Half life
Olmesartan: Approximately 13 hours. Hydrochlorothiazide: 5.8 to 14.8 hours.
Absorption
Olmesartan: Rapidly absorbed, bioavailability approximately 26%. Hydrochlorothiazide: Rapidly absorbed, bioavailability approximately 65-75%.
Metabolism
Olmesartan: Negligible hepatic metabolism. Hydrochlorothiazide: Not metabolized.
Side Effects
Contraindications
- Hypersensitivity to Olmesartan, Hydrochlorothiazide, or any sulfonamide-derived drugs.
- Anuria (absence of urine production).
- Severe renal impairment (creatinine clearance < 30 mL/min).
- Severe hepatic impairment or biliary obstruction.
- Pregnancy (especially second and third trimesters).
- Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment.
Drug Interactions
Lithium
Increased serum lithium concentrations and lithium toxicity. Monitor lithium levels.
Aliskiren
Contraindicated in diabetic patients or those with renal impairment when used concurrently with Olmesartan.
Corticosteroids, ACTH
Increased electrolyte depletion, particularly hypokalemia.
Alcohol, Barbiturates, Narcotics
May potentiate orthostatic hypotension.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal impairment.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia. Monitor potassium levels.

Store below 30°C in a cool, dry place. Protect from light and moisture. Keep out of reach of children.

Symptoms of overdose include hypotension (low blood pressure), dizziness, tachycardia (rapid heartbeat), bradycardia (slow heartbeat), and electrolyte disturbances. Treatment is symptomatic and supportive. Olmesartan is not dialyzable, while hydrochlorothiazide is dialyzable.
Pregnancy & Lactation
Olmezest-H is contraindicated in the second and third trimesters of pregnancy due to the risk of fetal injury and death. It is not recommended during the first trimester. It is also not recommended for use during breastfeeding as components may pass into breast milk.
