Description
Overview of the medicine
Olopred-plus 05-03 Suspension is an oral medication combining Olopatadine, an H1-receptor antagonist and mast cell stabilizer, with Prednisolone, a potent corticosteroid. It is indicated for the symptomatic relief of allergic and inflammatory conditions.
Uses & Indications
Dosage
Adults
Usually 5-10 ml orally once or twice daily, or as directed by physician. Dosage may vary based on condition severity.
Elderly
Lower initial doses recommended; monitor for adverse effects.
Renal_impairment
Use with caution. Dosage adjustment may be necessary based on renal function.
How to Take
Administer orally. Shake the bottle well before each use. Can be taken with or without food.
Mechanism of Action
Olopatadine selectively blocks histamine H1 receptors, preventing the effects of histamine and stabilizes mast cells to inhibit the release of inflammatory mediators. Prednisolone, a glucocorticoid, acts by binding to specific intracellular glucocorticoid receptors, modifying gene expression to exert potent anti-inflammatory and immunosuppressive effects.
Pharmacokinetics
Onset
Olopatadine: Within 30-60 minutes. Prednisolone: Within 1-2 hours.
Excretion
Olopatadine: Primarily renal (65% unchanged drug and metabolites). Prednisolone: Primarily renal.
Half life
Olopatadine: 8-12 hours. Prednisolone: Plasma half-life 2-3 hours; biological half-life 18-36 hours.
Absorption
Olopatadine: Rapidly absorbed, peak plasma concentrations 1-2 hours. Prednisolone: Readily and rapidly absorbed, peak plasma concentrations 1-2 hours.
Metabolism
Olopatadine: Primarily by oxidation, minor CYP450. Prednisolone: Primarily hepatic to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Olopatadine, Prednisolone or any component of the formulation
- Systemic fungal infections
- Untreated bacterial, viral or parasitic infections
- Live attenuated vaccines during immunosuppressive doses of corticosteroids
Drug Interactions
Olopatadine
Concomitant use with alcohol or other CNS depressants may enhance drowsiness.
Prednisolone
CYP3A4 inducers (e.g., rifampin, phenobarbital) may decrease Prednisolone levels. CYP3A4 inhibitors (e.g., ketoconazole, ritonavir) may increase Prednisolone levels. Concomitant use with NSAIDs increases risk of GI ulceration. Live attenuated vaccines should not be given.
Storage
Store below 30°C, protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose may include increased side effects such as drowsiness, hypertension, fluid retention, or hyperglycemia. Management is primarily supportive and symptomatic. Gastric lavage or activated charcoal may be considered soon after acute ingestion.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Both components are excreted in breast milk; caution should be exercised when administered to a nursing mother.
