Description
Overview of the medicine
Olvance 20 mg Tablet contains Olmesartan Medoxomil, an angiotensin II receptor blocker (ARB), used to treat high blood pressure (hypertension). It helps to lower blood pressure by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
The usual recommended starting dose of Olmesartan Medoxomil is 20 mg once daily. The dose can be increased to 40 mg once daily if blood pressure is not adequately controlled after 2 weeks.
Elderly
No initial dose adjustment is required for elderly patients. However, caution should be exercised with dose increases.
Renal_impairment
Patients with moderate to severe renal impairment (creatinine clearance <40 mL/min) should be initiated with 10 mg once daily. Higher doses should be used with caution.
How to Take
Administer orally once daily with or without food. Swallow the tablet whole with water.
Mechanism of Action
Olmesartan is a selective AT1 subtype angiotensin II receptor antagonist. It blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT1 receptor in many tissues, including vascular smooth muscle and the adrenal gland. This leads to vasodilation, decreased peripheral resistance, and reduced blood pressure.

Absorption
Rapidly and completely converted to active olmesartan during absorption from the gastrointestinal tract. Bioavailability is approximately 26%. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
No significant metabolism by the cytochrome P450 system. It is hydrolyzed to active olmesartan in the GI tract.
Side Effects
Contraindications
- Hypersensitivity to Olmesartan Medoxomil or any component of the product.
- Pregnancy (especially second and third trimesters).
- Severe hepatic impairment.
- Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR <60 mL/min/1.73 m²).
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
Aliskiren
Increased risk of hypotension, hyperkalemia, and renal impairment (especially in diabetic or renally impaired patients).
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal impairment.
Potassium-sparing diuretics/potassium supplements
Increased risk of hyperkalemia.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms of overdose are likely to be hypotension and tachycardia; bradycardia could occur from parasympathetic stimulation. Management involves symptomatic and supportive treatment, including IV fluids and vasopressors if severe hypotension occurs.
