Description
Overview of the medicine
Omastin is a monoclonal antibody used in the treatment of various cancers. It works by targeting the PD-1 (programmed cell death-1) protein on T-cells, thereby enhancing the immune system's ability to fight cancer cells.
Uses & Indications
Dosage
Adults
200 mg administered intravenously over 30 minutes every 3 weeks, or 400 mg every 6 weeks, until disease progression or unacceptable toxicity.
Elderly
No specific dose adjustment required based on age; monitor for adverse reactions.
Renal_impairment
No dose adjustment recommended for mild to moderate renal impairment. Data for severe impairment is limited.
How to Take
Administer as an intravenous infusion over 30 minutes. Do not administer as an intravenous push or bolus.
Mechanism of Action
Omastinumab is an anti-PD-1 monoclonal antibody that blocks the interaction between PD-1 and its ligands, PD-L1 and PD-L2. This blockade releases PD-1 pathway-mediated inhibition of the immune response, including anti-tumor immune response, leading to tumor regression.
Pharmacokinetics
Onset
Immune-mediated effects can be seen within weeks, clinical response may take longer.
Excretion
Primarily through cellular uptake and degradation.
Half life
Approximately 2-3 weeks (14-21 days).
Absorption
Administered intravenously, leading to complete systemic bioavailability.
Metabolism
Catabolized via general protein degradation pathways; not metabolized by cytochrome P450 enzymes.
Side Effects
Contraindications
- Known hypersensitivity to Omastinumab or any excipients.
- Patients with severe active autoimmune disease requiring systemic immunosuppression.
Drug Interactions
Systemic Corticosteroids/Immunosuppressants
May interfere with Omastinumab's activity; avoid use prior to initiation of Omastinumab.
Storage
Store refrigerated at 2°C to 8°C (36°F to 46°F) in the original carton to protect from light. Do not freeze. Do not shake.
Overdose
There is no specific antidote for Omastinumab overdose. In case of an overdose, monitor patients closely for signs and symptoms of adverse reactions and provide symptomatic and supportive treatment.
Pregnancy & Lactation
Pregnancy Category D. Omastinumab can cause fetal harm. Advise pregnant women of the potential risk to the fetus. It is unknown if Omastinumab is excreted in human milk; due to the potential for serious adverse reactions in breastfed infants, advise women not to breastfeed during treatment and for 5 months after the last dose.
