Description
Overview of the medicine
Ometac 20 mg capsule contains Omeprazole, a proton pump inhibitor used to reduce the amount of acid produced in the stomach. It is effective in treating conditions like heartburn, acid reflux, and stomach ulcers.
Uses & Indications
Dosage
Adults
Usually 20 mg once daily. For severe cases or Zollinger-Ellison Syndrome, higher doses up to 40-60 mg daily may be prescribed. H. pylori eradication usually involves 20 mg twice daily with antibiotics for 7-14 days.
Elderly
No specific dosage adjustment is required unless there is severe hepatic impairment.
Renal_impairment
No dosage adjustment is generally necessary for patients with impaired renal function.
How to Take
Take the capsule orally, preferably in the morning, before a meal. Swallow the capsule whole with water; do not chew or crush it. If you have difficulty swallowing, open the capsule and mix the contents with a small amount of fruit juice or applesauce and swallow immediately.
Mechanism of Action
Omeprazole works by irreversibly blocking the H+/K+-ATPase enzyme system (proton pump) located in the gastric parietal cells. This action effectively inhibits the final step of acid production in the stomach, leading to a significant and sustained reduction in gastric acid secretion.
Pharmacokinetics
Onset
Acid secretion inhibition begins within 1 hour, maximum effect within 2 hours.
Excretion
Approximately 77% of the dose is excreted as metabolites in the urine, and about 20% in the feces.
Half life
Plasma half-life is approximately 0.5 to 1 hour, but the duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 0.5 to 3.5 hours. Bioavailability increases with repeated doses.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 enzyme system (CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir (an antiviral drug).
Drug Interactions
Warfarin
Increased INR (International Normalized Ratio) and bleeding risk.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased methotrexate levels, especially with high doses.
Nelfinavir/Atazanavir
Reduced plasma concentrations and efficacy of these antiretrovirals.
Diazepam, Phenytoin, Citalopram
Increased plasma levels of these drugs.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, dizziness, abdominal pain, headache, lethargy, and tachycardia. No specific antidote is known. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C (old system). Studies have not shown an increased risk of birth defects. Use in pregnancy only if clearly needed. Omeprazole is excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but caution is advised.

Lab Monitoring
- Long-term use may require periodic monitoring of Vitamin B12 levels.
- Magnesium levels should be monitored in patients expected to be on prolonged treatment, or who take omeprazole with other drugs that may cause hypomagnesemia.
Doctor Notes
- Emphasize taking before food for optimal effect.
- Advise patients on lifestyle modifications to complement therapy.
- For long-term use, discuss risks of C. difficile, hypomagnesemia, and bone fracture, and consider lowest effective dose.
Patient Guidelines
- Take exactly as prescribed by your doctor.
- Swallow the capsule whole; do not chew, crush, or break it.
- Take before food, usually in the morning.
- Inform your doctor about all other medications, supplements, and herbal products you are taking.
- Do not stop taking the medicine suddenly without consulting your doctor.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Omeprazole is generally not expected to affect the ability to drive or use machines. However, some side effects like dizziness or visual disturbances may occur rarely. If you experience such symptoms, avoid driving or operating machinery.
Lifestyle Advice
- Avoid foods and beverages that trigger acid reflux (e.g., spicy foods, caffeine, fatty foods).
- Elevate the head of your bed if you experience nighttime heartburn.
- Avoid smoking and limit alcohol consumption.
