Description
Overview of the medicine
Omeprazole is a proton pump inhibitor (PPI) that decreases the amount of acid produced in the stomach. It is used to treat and prevent stomach and esophageal problems such as acid reflux, ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For Duodenal ulcer: 20 mg once daily for 2-4 weeks. For Gastric ulcer: 20 mg once daily for 4-8 weeks. For GERD: 20 mg once daily for 4-8 weeks. For Zollinger-Ellison syndrome: 60 mg once daily, adjusted as needed.
Elderly
No specific dose adjustment is generally required, but caution is advised.
Renal_impairment
No dose adjustment necessary.
How to Take
Take orally, preferably in the morning before food. Capsules should be swallowed whole with water, not chewed or crushed.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the parietal cells of the stomach, thereby reducing gastric acid secretion.

Excretion
Approximately 80% excreted in urine as metabolites, and the remainder in feces.
Half life
0.5-1 hour (plasma); 24-72 hours (acid secretion inhibition)
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is about 30-40% after a single dose and increases with repeated administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 enzyme system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to omeprazole or any component of the formulation
- •Concomitant use with nelfinavir
Drug Interactions
Diazepam
Prolonged elimination of diazepam.
Warfarin
Increased INR/prothrombin time.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to CYP2C19 inhibition.
Methotrexate
Increased plasma concentration of methotrexate (high doses).
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, confusion, blurred vision, tachycardia, nausea, vomiting, headache, dry mouth. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy category C; use only if clearly needed. Excreted in breast milk, use with caution during lactation.
