Description
Overview of the medicine
Ometop 20 mg Capsule contains Omeprazole, a proton pump inhibitor used to reduce stomach acid production. It is used to treat conditions like heartburn, acid reflux, stomach ulcers, and certain other digestive issues.
Uses & Indications
Dosage
Adults
Typical dosage is 20-40 mg once daily, usually in the morning before a meal. Duration varies depending on the condition (e.g., 4-8 weeks for ulcers, long-term for Zollinger-Ellison Syndrome).
Elderly
No specific dosage adjustment is generally required for elderly patients, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Take Ometop capsule orally, usually once a day in the morning, at least 30-60 minutes before a meal. Swallow the capsule whole with water; do not chew or crush it.
Mechanism of Action
Omeprazole irreversibly blocks the hydrogen-potassium adenosine triphosphatase (H+/K+-ATPase) enzyme system, also known as the proton pump, in the gastric parietal cells. This action effectively inhibits the final step in gastric acid secretion, leading to reduced stomach acidity.
Pharmacokinetics
Onset
Onset of acid secretion inhibition is within 1-2 hours.
Excretion
Approximately 80% of metabolites are excreted in the urine and the remainder in the feces.
Half life
Plasma half-life is approximately 0.5 to 1 hour, but the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Omeprazole is rapidly absorbed after oral administration, with peak plasma concentrations occurring within 0.5 to 3.5 hours. Bioavailability is approximately 30-40% after a single oral dose but increases to about 60% with repeated administration due to saturable first-pass metabolism.
Metabolism
Extensively metabolized in the liver primarily by the cytochrome P450 system (CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to Omeprazole or any component of the formulation.
- Concomitant use with nelfinavir (due to significant reduction in nelfinavir plasma levels).
Drug Interactions
Digoxin
Increased systemic exposure to digoxin.
Diazepam
Delayed elimination of diazepam.
Warfarin
Increased INR/prothrombin time in patients receiving concomitant Omeprazole and Warfarin.
Phenytoin
Increase in phenytoin levels.
Tacrolimus
Increased serum levels of tacrolimus.
Clopidogrel
Omeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19.
Methotrexate
Possible increase in methotrexate levels, especially with high doses.
St. John's Wort
May reduce omeprazole exposure.
Ketoconazole/Itraconazole
Reduced absorption of these drugs due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and self-limiting, including nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. No specific antidote is known; treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Small amounts of omeprazole are excreted in breast milk; caution is advised when administered to a nursing mother. Consult a doctor.

- For long-term use (over one year), periodic monitoring of Vitamin B12 levels may be considered.
- Monitor magnesium levels, especially in patients on concomitant diuretics.
Doctor Notes
- Assess the need for long-term PPI therapy regularly; consider lowest effective dose.
- Be aware of potential drug interactions, especially with clopidogrel and warfarin.
- Educate patients on proper administration and potential long-term risks.
