Description
Overview of the medicine
Omeprazole 20 mg capsule is a proton pump inhibitor (PPI) used to treat conditions caused by too much stomach acid. It works by decreasing the amount of acid your stomach makes, thereby relieving symptoms like heartburn, difficulty swallowing, and persistent cough, and allowing the esophagus to heal.
Uses & Indications
Dosage
Adults
GERD: 20 mg once daily for 4-8 weeks. Duodenal ulcer: 20 mg once daily for 2-4 weeks. Gastric ulcer: 20 mg once daily for 4-8 weeks. Zollinger-Ellison syndrome: Initial 60 mg once daily, adjust as needed. H. pylori eradication: 20 mg twice daily with antibiotics for 7-14 days.
Elderly
No dosage adjustment is generally required, but caution is advised due to potential age-related decrease in elimination.
Renal_impairment
No dosage adjustment is required for patients with impaired renal function.
How to Take
Take the capsule whole with a glass of water, preferably in the morning before a meal. Do not chew or crush the capsule.
Mechanism of Action
Omeprazole is a substituted benzimidazole that suppresses gastric acid secretion by specific inhibition of the H+/K+-ATPase enzyme system (the proton pump) at the secretory surface of the gastric parietal cells. It is a prodrug that is activated in the acidic environment of the parietal cell canaliculi.

Absorption

Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to omeprazole or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Diazepam
Decreased metabolism and increased plasma levels of diazepam.
Warfarin
Increased INR/prothrombin time; monitor closely.
Clopidogrel
Omeprazole can reduce the antiplatelet effect of clopidogrel.
Methotrexate
Increased plasma levels of methotrexate, especially with high doses.
Atazanavir, Nelfinavir
Reduced absorption and efficacy of these antiretrovirals.

Overdose
Symptoms of overdose are generally mild and reversible, including nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. Treatment is symptomatic and supportive.

Pregnancy Category C. Use in pregnancy only if potential benefit justifies potential risk to the fetus. Excreted in breast milk; use with caution in lactating mothers.
