Overview of the medicine
Ondansetron is an antiemetic used to prevent nausea and vomiting caused by chemotherapy, radiation therapy, and surgery. It works by blocking the action of serotonin, a natural substance in the body that can cause nausea and vomiting.
Uses & Indications
Dosage
Adults
For highly emetogenic chemotherapy: 8 mg oral solution twice daily for up to 5 days. For moderately emetogenic chemotherapy/radiotherapy: 8 mg oral solution once daily or 8 mg twice daily, depending on regimen. For PONV: 4 mg oral solution as a single dose.
Elderly
No specific dose adjustment is generally required. However, for patients over 75 years, the initial intravenous dose should not exceed 8 mg. Oral dosage typically remains similar.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Onatron oral solution can be taken with or without food. Measure the dose accurately using the provided measuring syringe or cup. Follow your doctor's instructions regarding dose and duration.
Mechanism of Action
Ondansetron is a selective 5-HT3 receptor antagonist. It blocks serotonin, both peripherally on vagal nerve terminals and centrally in the chemoreceptor trigger zone, thereby preventing the emetic reflex.

Onset
Within 30-60 minutes after oral administration.
Excretion
Approximately 5% of the dose is excreted unchanged in urine, with the remainder excreted as metabolites via urine and feces.
Half life
Approximately 3-6 hours in adults, may be prolonged in elderly and those with severe hepatic impairment.
Absorption
Well absorbed orally, peak plasma concentrations reached within 1.5-2.5 hours. Bioavailability is approximately 50-70%.
Metabolism
Extensive hepatic metabolism, primarily by CYP1A2, CYP2D6, and CYP3A4, followed by glucuronide and sulfate conjugation.
Side Effects
Contraindications
- •Hypersensitivity to ondansetron or any component of the formulation.
- •Concomitant use of apomorphine with ondansetron due to the risk of severe hypotension and loss of consciousness.
Drug Interactions
Tramadol
Ondansetron may reduce the analgesic effect of tramadol.
Apomorphine
Concomitant use is contraindicated due to reports of profound hypotension and loss of consciousness.
Phenytoin, Carbamazepine, Rifampicin
These strong CYP3A4 inducers may reduce plasma concentrations of ondansetron.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep the bottle tightly closed and out of reach of children.
Overdose
Symptoms of overdose include visual disturbances, severe constipation, hypotension, and vasovagal episodes with transient AV block. Treatment is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm, but adequate human studies are lacking. Use only if clearly needed. Ondansetron is excreted in breast milk; caution is advised during breastfeeding.
