Description
Overview of the medicine
Onbrez 300 mcg is an inhalation capsule containing Indacaterol, a long-acting beta2-adrenergic agonist (LABA), used for the maintenance treatment of chronic obstructive pulmonary disease (COPD). It is not for the relief of acute bronchospasm.
Uses & Indications
Dosage
Adults
One 300 mcg capsule inhaled once daily using the Onbrez Breezhaler device. Do not swallow the capsule.
Elderly
No dosage adjustment is required in elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
For oral inhalation only, using the specific Onbrez Breezhaler device. The capsule must not be swallowed. Administer at the same time each day.
Mechanism of Action
Indacaterol selectively stimulates beta2-adrenergic receptors in the lung, leading to relaxation of bronchial smooth muscle and bronchodilation. It provides 24-hour bronchodilation when administered once daily.
Pharmacokinetics
Onset
Within 5 minutes
Excretion
Mainly via feces (90%), with a smaller portion excreted renally (10%).
Half life
Approximately 40-56 hours
Absorption
Rapidly absorbed after inhalation. Peak plasma concentrations are reached within 15-30 minutes.
Metabolism
Primarily via UGT1A1 and to a lesser extent CYP3A4. Main metabolite is hydroxylated indacaterol.
Side Effects
Contraindications
- Hypersensitivity to indacaterol or any excipients
- Not for the treatment of acute bronchospasm or status asthmaticus
- Avoid use in patients with severe milk protein allergy due to lactose content
Drug Interactions
Beta-blockers
May reduce the bronchodilatory effect of indacaterol and should be used with caution, preferably cardioselective beta-blockers.
Other sympathomimetics
May potentiate adverse cardiovascular effects.
Non-potassium-sparing diuretics
May increase the risk of hypokalemia.
MAO inhibitors, tricyclic antidepressants
May potentiate the action of indacaterol on the cardiovascular system.
Storage
Store in a dry place below 30°C. Protect from moisture and heat. Keep out of reach of children.
Overdose
Symptoms of overdose may include exaggeration of typical beta2-adrenergic effects, such as tachycardia, tremors, palpitations, headache, nausea, vomiting, drowsiness, and hypokalemia. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Category C (old classification). Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Use with caution during lactation; it is unknown if indacaterol is excreted in human milk.
