Overview of the medicine
Ondansetron is an antiemetic used to prevent nausea and vomiting caused by chemotherapy, radiation therapy, and surgery.
Uses & Indications
Dosage
Adults
Chemotherapy-induced nausea and vomiting: 8-24 mg orally or intravenously, depending on emetogenicity. Post-operative nausea and vomiting: 16 mg orally one hour before anesthesia, or 4 mg IV/IM at induction.
Elderly
No general dose adjustment required, but caution in patients over 75 years due to risk of QT prolongation.
Renal_impairment
No dosage adjustment needed for patients with renal impairment.
How to Take
Oral tablets can be taken with or without food. Oral solution can be measured with a dosing syringe. Injections are administered intravenously or intramuscularly by a healthcare professional.
Mechanism of Action
It works by blocking serotonin, a natural substance in the body that can cause nausea and vomiting.

Onset
Approximately 30 minutes (oral), a few minutes (IV).
Excretion
About 5% is excreted unchanged in urine, with the remainder excreted as metabolites via urine and feces.
Half life
Approximately 3-6 hours in adults, longer in elderly and renally/hepatically impaired patients.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are reached within 1.5 hours.
Metabolism
Extensively metabolized in the liver by multiple cytochrome P450 enzymes (CYP1A2, CYP2D6, CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to ondansetron or any component of the formulation.
- Concomitant use with apomorphine.
Drug Interactions
Tramadol
Increased risk of serotonin syndrome.
Apomorphine
Concomitant use is contraindicated due to severe hypotension and loss of consciousness.
QT-prolonging drugs
Increased risk of QT prolongation and Torsade de Pointes.
Phenytoin, Carbamazepine, Rifampicin
May decrease ondansetron levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose may cause visual disturbances, severe constipation, hypotension, and vasovagal episodes. Management includes symptomatic and supportive treatment, including ECG monitoring.
Pregnancy & Lactation
Pregnancy Category B. Studies in pregnant animals have not shown a risk to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during lactation should be with caution.
