Uses & Indications
Dosage
Adults
Though Onefix DS is primarily for children, the adult dose for Cefixime is typically 200 mg to 400 mg daily, given as a single dose or in two divided doses.
Children
Recommended dose is 8 mg/kg/day, administered as a single dose or in two divided doses every 12 hours. For children weighing over 50 kg or older than 12 years, the adult dose is recommended.
Renal_impairment
Dosage adjustment is necessary for patients with severe renal impairment (creatinine clearance < 30 ml/min). The dose should be reduced to 4 mg/kg/day.
How to Take
Onefix DS is an oral suspension. Reconstitute the powder with boiled and cooled water as per instructions on the label. Shake the bottle well before each use. Can be taken with or without food. Complete the full course of treatment as prescribed by the doctor.
Mechanism of Action
Cefixime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains that are essential for bacterial cell wall strength and rigidity. This leads to cell lysis and bacterial death.

Peak plasma concentrations are achieved 2-6 hours after administration.
Excretion
Primarily excreted by the kidneys (about 50% of the absorbed dose is excreted unchanged in urine within 24 hours), and also through biliary excretion.
Half life
The plasma half-life is typically 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Absorption is not significantly altered by food.
Metabolism
Cefixime is not extensively metabolized in the body; a small amount undergoes metabolism in the gut.
