Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor that decreases the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), stomach ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. H. pylori: 20 mg twice daily for 10 days (with antibiotics). Zollinger-Ellison Syndrome: 40-80 mg twice daily.
Elderly
Generally, no dose adjustment is required for elderly patients.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take at least one hour before a meal, swallow the capsule whole. Do not chew or crush.
Mechanism of Action
Irreversibly inhibits the H+/K+-ATPase proton pump in gastric parietal cells, thereby reducing gastric acid secretion.

Excretion
Mainly renal (approx. 80%) and fecal (approx. 20%).
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed. Bioavailability increases with repeated dosing.
Metabolism
Extensively hepatic via CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
May increase INR and bleeding risk.
Clopidogrel
May decrease the antiplatelet effect of clopidogrel.
Methotrexate
May increase methotrexate levels and toxicity.
Atazanavir/Nelfinavir
May significantly decrease the plasma levels of these antiretrovirals.
Storage
Store below 30°C, in a dry place, protected from light and moisture.
Overdose
Treatment of overdose is symptomatic and supportive. Esomeprazole is extensively protein bound and not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is unknown if esomeprazole is excreted in human milk; use with caution in nursing mothers.
