Description
Overview of the medicine
Omeprazole is a proton pump inhibitor (PPI) used to treat conditions caused by too much stomach acid. It helps heal acid-related damage to the stomach and esophagus, and prevents ulcers.
Uses & Indications
Dosage
Adults
Typical dose for GERD is 20 mg once daily for 4-8 weeks. For duodenal ulcer, 20 mg once daily for 2-4 weeks. For Zollinger-Ellison syndrome, 60 mg once daily, adjusted as needed.
Elderly
No specific dose adjustment is required for elderly patients, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take orally, preferably in the morning, at least 30 minutes before food. Swallow the capsule whole with water; do not chew or crush.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells. This reduces gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour; maximal acid suppression in 2 hours.
Excretion
Mainly excreted in urine (approx. 77%) and feces (approx. 20%).
Half life
Plasma elimination half-life is 0.5 to 1 hour, but duration of acid suppression is much longer (up to 72 hours).
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability increases with repeated dosing.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Known hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased digoxin levels.
Warfarin
Increased INR and prothrombin time.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to inhibition of CYP2C19.
Methotrexate
Increased methotrexate levels and prolonged exposure.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and transient, including nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C (older classification, generally considered low risk for short-term use). Use during pregnancy only if clearly needed. Omeprazole is excreted in breast milk; caution should be exercised when administered to a nursing woman.
