Description
Overview of the medicine
Opegus 200 mg Capsule is a macrolide antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria, thereby clearing the infection. It is effective against various respiratory, skin, and soft tissue infections.
Uses & Indications
Dosage
Adults
The usual adult dose is 200 mg once daily for 7-10 days, depending on the severity and type of infection.
Elderly
No specific dose adjustment is generally required for elderly patients, but monitor for renal and hepatic function.
Renal_impairment
For mild to moderate renal impairment, no dose adjustment is necessary. For severe renal impairment (CrCl < 30 mL/min), consider a reduced dose or extended dosing interval under medical supervision.
How to Take
Take Opegus 200 mg Capsule orally with or without food. Swallow the capsule whole with a glass of water. Do not crush, chew, or break the capsule.
Mechanism of Action
Opegacin, the active ingredient in Opegus, inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible microorganisms. This action prevents the elongation of peptide chains, thus impeding bacterial growth and replication.
Pharmacokinetics
Onset
Therapeutic effects typically begin within 1-2 hours of administration.
Excretion
Mainly excreted through the bile (hepatic excretion), with a smaller portion excreted renally.
Half life
The elimination half-life is approximately 8-12 hours, allowing for once-daily dosing.
Absorption
Rapidly and well-absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are usually reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver, likely via the cytochrome P450 (CYP3A4) system.
Side Effects
Contraindications
- Hypersensitivity to Opegacin or any other macrolide antibiotic.
- History of cholestatic jaundice/hepatic dysfunction associated with previous macrolide use.
- Concomitant use with drugs known to prolong the QT interval (e.g., certain antiarrhythmics, antipsychotics).
Drug Interactions
Digoxin
May increase serum digoxin levels. Monitor digoxin levels and adjust dose as necessary.
Warfarin
Increased anticoagulant effect, leading to a higher risk of bleeding. Monitor INR closely.
Theophylline
Increased plasma levels of theophylline, potentially leading to toxicity. Monitor theophylline levels.
CYP3A4 inhibitors/inducers
Potential for altered Opegacin levels. Adjust dose as needed.
Statins (e.g., Simvastatin)
Increased risk of myopathy and rhabdomyolysis due to inhibition of CYP3A4 metabolism.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of an overdose, symptomatic and supportive measures should be taken. Gastric lavage and activated charcoal may be considered. Monitor vital signs and ECG (due to potential for QT prolongation). Hemodialysis is not effective in removing Opegacin from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Opegacin is excreted in breast milk; caution should be exercised when administered to a nursing mother.

Clinical Trials
Opegacin underwent extensive Phase I, II, and III clinical trials demonstrating its efficacy and safety profile. Post-marketing surveillance and Phase IV studies are ongoing to further evaluate its long-term effects and rare adverse events.
Lab Monitoring
- Liver function tests (LFTs) should be monitored, especially during prolonged therapy or in patients with pre-existing hepatic impairment.
- ECG monitoring is advisable in patients with known cardiac risk factors for QT prolongation.
Doctor Notes
- Prescribe Opegus only after confirming bacterial infection and considering local resistance patterns.
- Advise patients on the importance of completing the full course to prevent resistance.
- Monitor liver function tests periodically for patients on prolonged therapy or with hepatic concerns.
Patient Guidelines
- Complete the entire course of medication as prescribed, even if symptoms improve.
- Do not share your medication with others.
- Inform your doctor about all other medications you are taking, including over-the-counter drugs and herbal supplements.
Missed Dose Advice
If you miss a dose of Opegus, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Opegus may cause dizziness or visual disturbances in some patients. If you experience these effects, avoid driving or operating heavy machinery.
Lifestyle Advice
- Maintain adequate hydration by drinking plenty of fluids.
- Avoid alcohol consumption during the course of treatment as it may exacerbate side effects.
