Description
Overview of the medicine
Opiren 30 mg capsule contains Rabeprazole, which is a proton pump inhibitor (PPI) used to treat conditions where there is too much acid in the stomach. It works by irreversibly blocking the proton pump in the gastric parietal cells, thereby reducing gastric acid secretion.
Uses & Indications
Dosage
Adults
The usual adult dose for gastroesophageal reflux disease (GERD) is 20 mg once daily. For the 30 mg strength, it may be used for severe erosive esophagitis, Zollinger-Ellison syndrome, or when a higher dose is deemed necessary by a physician. For Zollinger-Ellison syndrome, the initial dose is typically 60 mg once daily, adjusted as needed. For H. pylori eradication, a specific 30 mg dose might be part of a combination regimen.
Elderly
No dosage adjustment is generally required for elderly patients, but caution is advised.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Take the capsule orally, preferably in the morning before breakfast. Swallow the capsule whole with water; do not chew or crush it.
Mechanism of Action
Rabeprazole belongs to a class of antisecretory compounds, the substituted benzimidazoles, that do not exhibit anticholinergic or H2-histamine antagonistic properties, but suppress gastric acid secretion by inhibiting the gastric H+/K+-ATPase (the proton pump) at the secretory surface of the gastric parietal cell. This effect is dose-related and leads to inhibition of both basal and stimulated acid secretion.
Pharmacokinetics
Onset
Within 1 hour of administration.
Excretion
Approximately 90% of a dose is excreted in the urine, predominantly as metabolites, and 10% is excreted in the feces.
Half life
Plasma elimination half-life is approximately 1-2 hours in healthy subjects, independent of dose. However, the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring approximately 2-3.5 hours post-dose. Absolute bioavailability is approximately 52% due to presystemic metabolism. Food and time of administration do not significantly affect absorption.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 (CYP) isoenzymes CYP2C19 and CYP3A4. Its main metabolites are thioether and sulfone.
Side Effects
Contraindications
- Hypersensitivity to rabeprazole, substituted benzimidazoles, or any component of the formulation.
- Not recommended for immediate relief of heartburn.
Drug Interactions
Digoxin
May increase digoxin absorption and plasma concentrations.
Warfarin
Increased INR and prothrombin time; monitor closely.
Clopidogrel
Reduced antiplatelet effect of clopidogrel. Avoid concomitant use.
Methotrexate
Increased and prolonged plasma concentrations of methotrexate; monitor for toxicity.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
There is limited experience with overdosage. Symptoms may include drowsiness, headache, nausea, vomiting, and diarrhea. In the event of overdose, treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown evidence of impaired fertility or harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Rabeprazole and/or its metabolites are excreted in breast milk in rats, but it is not known whether rabeprazole is excreted in human milk. Caution should be exercised when administered to a nursing woman.
