Description
Overview of the medicine
Optadex-01-05 Eye Drop is a combination medicine containing an antibiotic (Tobramycin) and a corticosteroid (Dexamethasone). It is used to treat bacterial eye infections where inflammation is also present.
Uses & Indications
Dosage
Adults
Instill 1-2 drops into the affected eye(s) every 4 to 6 hours. For severe infections, dosage may be increased to 1-2 drops every 2 hours for the first 24 to 48 hours, then decreased as the infection resolves.
Elderly
No specific dosage adjustment is generally required for elderly patients.
Renal_impairment
No specific dosage adjustment is necessary for patients with renal impairment due to minimal systemic absorption from ophthalmic administration.
How to Take
For ophthalmic use only. Shake the bottle well before each use. To avoid contamination, do not touch the dropper tip to any surface. Apply gentle pressure to the tear duct after instilling drops to minimize systemic absorption. Remove contact lenses before administration and wait at least 15 minutes before reinserting.
Mechanism of Action
Tobramycin is an aminoglycoside antibiotic that works by inhibiting bacterial protein synthesis, thus killing susceptible bacteria. Dexamethasone is a potent corticosteroid that suppresses the inflammatory response by reducing the release of inflammatory mediators.
Pharmacokinetics
Onset
Relatively rapid onset of action for both antibiotic and anti-inflammatory effects.
Excretion
Systemically absorbed tobramycin is primarily excreted unchanged by the kidneys. Ophthalmic excretion is local clearance.
Half life
Systemic half-life of tobramycin is 2-3 hours; not well characterized for topical ophthalmic use.
Absorption
Minimal systemic absorption occurs following ophthalmic administration, primarily acting locally.
Metabolism
Minimal metabolism for ophthalmic application; if absorbed systemically, tobramycin is not significantly metabolized.
Side Effects
Contraindications
- Hypersensitivity to any component of the medication.
- Acute untreated purulent ocular infections.
- Most viral diseases of the cornea and conjunctiva, including epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, and varicella.
- Mycobacterial infections of the eye.
- Fungal diseases of ocular structures.
Drug Interactions
Systemic Aminoglycosides
Although systemic absorption is minimal, concurrent use with systemic aminoglycosides should be monitored for increased serum levels.
Other Topical Ocular Medications
If more than one topical ophthalmic medication is being used, they should be administered at least 5 minutes apart.
Storage
Store at room temperature (below 30°C), away from direct sunlight and moisture. Do not freeze. Keep out of reach of children.
Overdose
An acute ophthalmic overdose is unlikely to cause systemic toxicity due to the limited absorption. If swallowed, supportive care and observation may be necessary. Local adverse effects such as punctate keratitis, erythema, increased lacrimation, and eyelid edema may occur with excessive topical application.
Pregnancy & Lactation
Use during pregnancy or lactation only if the potential benefit justifies the potential risk to the fetus or infant. Consult your doctor.

Clinical Trials
Clinical studies have demonstrated the efficacy and safety of tobramycin and dexamethasone combination eye drops in treating bacterial ocular infections with associated inflammation, showing superior outcomes compared to monotherapy.
Lab Monitoring
- For prolonged use (more than 10 days), regular monitoring of intraocular pressure (IOP) is recommended.
- Periodic slit-lamp examination is advisable to detect any corneal changes or secondary infections.
Doctor Notes
- Confirm bacterial etiology before prescribing to avoid steroid use in non-bacterial infections.
- Monitor intraocular pressure regularly, especially if prescribing for more than 10 days.
- Consider culture and sensitivity testing if the infection does not respond to initial treatment.
- Educate patients on proper administration technique and importance of completing the full course.
Patient Guidelines
- Complete the full course of treatment as prescribed by your doctor, even if symptoms improve.
- Do not wear contact lenses while using this medication. Wait at least 15 minutes after instilling drops before reinserting lenses.
- Avoid touching the dropper tip to your eye or any other surface to prevent contamination.
- If you are using other eye medications, administer them at least 5 minutes apart.
Missed Dose Advice
If you miss a dose, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not apply a double dose to make up for a missed one.
Driving Precautions
This medication may cause temporary blurred vision or other visual disturbances immediately after administration. If this occurs, avoid driving or operating machinery until your vision is clear.
Lifestyle Advice
- Maintain good eye hygiene to prevent further infections.
- Avoid rubbing your eyes excessively, as this can worsen irritation or spread infection.
