Overview of the medicine
Optimox-XG eye drop is a combination medication containing a fluoroquinolone antibiotic (Moxifloxacin) and a potent synthetic corticosteroid (Dexamethasone). It is used to treat bacterial eye infections where inflammation is also present, providing both antibacterial and anti-inflammatory effects.
Uses & Indications
Dosage
Adults
Instill one drop into the affected eye(s) 2-4 times daily. The duration of treatment should be determined by the physician, typically 5-7 days. Do not discontinue prematurely.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is considered necessary for patients with renal impairment due to minimal systemic absorption.
How to Take
For ophthalmic use only. Shake well before use. Tilt head back, pull down the lower eyelid to form a pouch, and instill the drop. Close eye gently and apply pressure to the inner corner of the eye for 1-2 minutes to prevent drainage. Do not touch the dropper tip to any surface.
Mechanism of Action
Moxifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death. Dexamethasone is a corticosteroid that suppresses the inflammatory response by inhibiting multiple inflammatory mediators and decreasing capillary permeability, thereby reducing swelling, redness, and pain.

Anti-inflammatory effects (Dexamethasone) can be felt within hours; antibacterial effects (Moxifloxacin) develop over 1-2 days.
Excretion
Mainly renal and fecal (if significant systemic absorption occurs).

Ocular half-life is short; systemic half-life, if absorbed: Moxifloxacin ~12 hours, Dexamethasone ~3-4.5 hours.
Absorption
Minimal systemic absorption after topical ophthalmic administration. Peak concentrations are achieved in ocular tissues within hours.

Contraindications

Overdose
