Description
Overview of the medicine
Orbas-Plus 20 mg tablet is a combination medication used to treat high blood pressure (hypertension). It contains Olmesartan Medoxomil, an angiotensin II receptor blocker, and Hydrochlorothiazide, a thiazide diuretic. This combination helps to lower blood pressure by relaxing blood vessels and increasing the excretion of water and sodium from the body.
Uses & Indications
Dosage
Adults
Typically one tablet (Olmesartan 20 mg / Hydrochlorothiazide 12.5 mg) once daily. Dosage may be increased to a maximum of Olmesartan 40 mg / Hydrochlorothiazide 25 mg if needed.
Elderly
No specific dose adjustment is generally required, but caution is advised, especially in those with impaired renal function.
Renal_impairment
Initial dose adjustments may be necessary for patients with moderate renal impairment (creatinine clearance 30-60 mL/min). Not recommended for severe renal impairment (creatinine clearance < 30 mL/min).
How to Take
Take orally once daily with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Olmesartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT1 receptor. Hydrochlorothiazide inhibits sodium and chloride reabsorption in the distal convoluted tubule, increasing the excretion of sodium, chloride, and water.

Onset
Within 1-2 hours for blood pressure lowering, maximal effect within 2-4 weeks.
Excretion
Olmesartan: Approximately 35-50% excreted in urine, remainder in feces. Hydrochlorothiazide: Primarily excreted unchanged in urine.
Half life
Olmesartan: 13 hours. Hydrochlorothiazide: 5.8 to 14.8 hours.
Absorption
Olmesartan: Rapidly absorbed and rapidly converted to olmesartan. Absolute bioavailability is approximately 26%. Hydrochlorothiazide: Rapidly absorbed, absolute bioavailability is 65-75%.
Metabolism
Olmesartan: Not metabolized by the cytochrome P450 system. Hydrochlorothiazide: Not metabolized.
Side Effects
Contraindications
- Hypersensitivity to Olmesartan, hydrochlorothiazide, or any sulfonamide-derived drugs.
- Anuria (absence of urine production) or severe renal impairment.
- Pregnancy (especially second and third trimesters).
- Severe hepatic impairment, biliary obstruction.
Drug Interactions
NSAIDs
Reduced antihypertensive effect; risk of renal impairment.
Lithium
Increased serum lithium concentrations and toxicity.
Other antihypertensives
Additive hypotensive effects.
Colestyramine/colestipol
Reduced absorption of hydrochlorothiazide.

Contraindicated in pregnancy, especially during the second and third trimesters, due to risks of fetal injury and death. Not recommended during lactation as both components are excreted in human milk and may cause adverse effects in a nursing infant.
