Description
Overview of the medicine
Orbidex-01 Eye Drop is a sterile ophthalmic solution containing a broad-spectrum fluoroquinolone antibiotic (Moxifloxacin) and a potent corticosteroid (Dexamethasone) used to treat bacterial infections of the eye and reduce associated inflammation. It is effective against a wide range of gram-positive and gram-negative bacteria.
Uses & Indications
Dosage
Adults
Instill 1 drop into the affected eye(s) 2-4 times daily for up to 7 days, or as directed by a physician. Shake well before use.
Elderly
No special dosage adjustment required for elderly patients.
Renal_impairment
No dosage adjustment required for renal impairment as systemic absorption is minimal.
How to Take
For ophthalmic use only. Tilt head back, pull down the lower eyelid to form a pocket, and instill the drop. Close eye gently. Avoid touching the dropper tip to the eye or any other surface to prevent contamination. If using other eye drops, wait at least 5 minutes between applications.
Mechanism of Action
Moxifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death. Dexamethasone is a potent synthetic corticosteroid that suppresses inflammation by inhibiting phospholipase A2 and reducing the release of inflammatory mediators.
Pharmacokinetics
Onset
Antibacterial effect typically within hours; anti-inflammatory effect within days.
Excretion
Excreted primarily through urine and feces for systemically absorbed components.
Half life
Systemic half-life of moxifloxacin is approximately 11-15 hours; topical ocular half-life is shorter due to local elimination.
Absorption
Minimal systemic absorption after topical ophthalmic administration. Peak concentrations in aqueous humor are achieved within 0.5-2 hours.
Metabolism
Mainly hepatic metabolism for systemically absorbed moxifloxacin; minimal for topical ophthalmic use. Dexamethasone is also hepatically metabolized.
Side Effects
Contraindications
- Hypersensitivity to Moxifloxacin, Dexamethasone, or any components of the formulation.
- Viral diseases of the cornea and conjunctiva (e.g., herpes simplex keratitis).
- Mycobacterial eye infection.
- Fungal infections of ocular structures.
Drug Interactions
Systemic corticosteroids
Increased risk of systemic side effects if significant absorption occurs (rare with eye drops).
Other topical ophthalmic medications
Allow at least 5 minutes between applications.
Storage
Store at room temperature (below 30°C). Protect from light and moisture. Keep out of reach of children.
Overdose
Topical ophthalmic overdose is unlikely to cause systemic toxicity due to limited absorption. If accidental ingestion occurs, symptomatic and supportive measures are recommended.
Pregnancy & Lactation
Use in pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Dexamethasone and Moxifloxacin are excreted in breast milk; caution should be exercised when administered to a nursing woman.
