Description
Overview of the medicine
Cefixime is an oral third-generation cephalosporin antibiotic used to treat various bacterial infections including those of the urinary tract, respiratory tract, otitis media, and gonorrhoea.
Uses & Indications
Dosage
Adults
The usual adult dose is 400 mg once daily or 200 mg every 12 hours. For uncomplicated gonorrhea, a single dose of 400 mg.
Elderly
No dosage adjustment usually required unless severe renal impairment (creatinine clearance < 60 mL/min).
Renal_impairment
For creatinine clearance 21-60 mL/min, the dose should be reduced to 75% of the standard dose. For creatinine clearance < 20 mL/min, the dose should be reduced to 50% of the standard dose.
How to Take
Orgacef 250 mg capsule can be taken with or without food. It should be swallowed whole with water.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. It is bactericidal.

Onset
1-2 hours (peak plasma concentration)
Excretion
Primarily excreted unchanged in urine (about 50%) and bile (about 10%).
Half life
Average 3-4 hours (ranging from 2-6 hours)
Absorption
Approximately 40-50% absorbed orally. Food may delay absorption but does not significantly affect the extent of absorption.
Metabolism
Limited hepatic metabolism; primarily excreted as unchanged drug.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of serious hypersensitivity reactions (e.g., anaphylaxis) to penicillin or other beta-lactam antibiotics.
Drug Interactions
Nifedipine
May increase Cefixime absorption.
Carbamazepine
Concomitant use with cefixime has been shown to increase carbamazepine levels.
Anticoagulants (e.g., Warfarin)
May increase prothrombin time (INR); careful monitoring and dosage adjustment of anticoagulants may be necessary.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Hemodialysis and peritoneal dialysis are generally not effective in removing cefixime. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; use with caution in nursing mothers.
