Description
Overview of the medicine
Oricox 120 mg tablet contains Etoricoxib, a selective cyclooxygenase-2 (COX-2) inhibitor, used for the symptomatic relief of osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and for the treatment of acute pain.
Uses & Indications
Dosage
Adults
Acute gouty arthritis: 120 mg once daily. Post-operative dental surgery pain: 120 mg once daily for a maximum of 8 days. Other indications: Typically 60 mg or 90 mg once daily, as directed by physician.
Elderly
No dose adjustment is generally necessary for elderly patients, but caution should be exercised due to potential age-related decrease in renal function.
Renal_impairment
No dose adjustment is necessary for patients with mild renal impairment. Caution is advised in patients with moderate renal impairment (creatinine clearance 30-60 mL/min). Not recommended for severe renal impairment (creatinine clearance <30 mL/min).
How to Take
Oricox 120 mg tablet can be taken orally, with or without food. It should be swallowed whole with water.
Mechanism of Action
Etoricoxib selectively inhibits cyclooxygenase-2 (COX-2), an enzyme responsible for the synthesis of prostaglandins, which are mediators of pain and inflammation. By inhibiting COX-2, etoricoxib reduces prostaglandin levels, leading to anti-inflammatory, analgesic, and antipyretic effects.
Pharmacokinetics
Onset
Pain relief typically begins within 24 minutes for dental pain and within 1 hour for other acute pains.
Excretion
Approximately 70% of the dose is excreted in the urine (as metabolites) and 20% in the feces (as metabolites).
Half life
The elimination half-life is approximately 22 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations (Cmax) reached approximately 1 hour after oral administration.
Metabolism
Extensively metabolized by the liver, primarily via cytochrome P450 (CYP) enzymes, particularly CYP3A4, to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to etoricoxib or any component of the tablet
- Active peptic ulceration or gastrointestinal (GI) bleeding
- Inflammatory bowel disease
- Severe hepatic dysfunction
- Severe renal impairment (creatinine clearance <30 mL/min)
- Congestive heart failure (NYHA Class II-IV)
- Established ischemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
- Uncontrolled hypertension (blood pressure consistently above 140/90 mmHg)
Drug Interactions
Lithium
Increased plasma lithium levels; monitor lithium levels.
Warfarin
Increased risk of bleeding; monitor INR.
Diuretics
Reduced diuretic and antihypertensive effects.
Rifampicin
Decreased etoricoxib plasma concentrations.
Methotrexate
Increased methotrexate toxicity; caution, especially with renal impairment.
ACE inhibitors/ARBs
Reduced antihypertensive effect and increased risk of renal impairment.
Oral Contraceptives
Increased plasma concentrations of ethinylestradiol, potentially increasing adverse effects.
Cyclosporine/Tacrolimus
Increased nephrotoxicity of cyclosporine/tacrolimus.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive measures should be taken. There is no specific antidote for etoricoxib. Dialysis is unlikely to be effective in removing etoricoxib due to its high protein binding.
Pregnancy & Lactation
Not recommended during pregnancy, especially in the third trimester, due to potential risk of premature closure of the ductus arteriosus and impaired fetal renal function. Etoricoxib is excreted in breast milk; therefore, its use is not recommended during lactation.
