Description
Overview of the medicine
Ornova (Metronidazole) is an antibiotic effective against a wide range of anaerobic bacteria and protozoa. It is used to treat various infections including amoebiasis, giardiasis, trichomoniasis, and bacterial vaginosis.
Uses & Indications
Dosage
Adults
Typically 400 mg three times daily for 5-10 days depending on the infection. For amoebiasis: 400-800 mg three times daily for 5-10 days.
Elderly
Dosage adjustment may be necessary in patients with severe hepatic impairment; otherwise, adult dosage.
Renal_impairment
No routine dosage adjustment needed in mild to moderate renal impairment. In severe impairment (CrCl < 10 mL/min), reduce dose by 50%.
How to Take
Orally with food or milk to minimize gastrointestinal upset. Swallow tablets whole.
Mechanism of Action
Metronidazole enters the organism and is reduced by anaerobic electron transport proteins to reactive nitro-radicals. These radicals damage DNA and other macromolecules, leading to cell death.
Pharmacokinetics
Onset
Therapeutic effects begin within a few hours.
Excretion
Mainly via urine (60-80%), with a small amount in feces.
Half life
Approximately 8 hours.
Absorption
Well absorbed orally, peak plasma concentrations occur within 1-2 hours.
Metabolism
Primarily hepatic, mainly by oxidation with side-chain cleavage.
Side Effects
Contraindications
- Hypersensitivity to metronidazole or other nitroimidazole derivatives
- First trimester of pregnancy (relative contraindication)
Drug Interactions
Alcohol
Disulfiram-like reaction (nausea, vomiting, flushing, tachycardia).
Lithium
Increases lithium levels, risk of toxicity.
Warfarin
Enhances anticoagulant effect, requiring INR monitoring.
Cimetidine
May increase metronidazole levels by reducing hepatic metabolism.
Phenobarbital/Phenytoin
May decrease metronidazole levels by inducing hepatic metabolism.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, ataxia, seizures, and peripheral neuropathy. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy category B. Avoid in the first trimester. Use with caution during lactation; may excrete into breast milk.
