Description
Overview of the medicine
Orpa-Plus 500 mg Tablet is a combination medicine containing Omeprazole and Metronidazole, primarily used for the treatment of Helicobacter pylori infection and related gastrointestinal disorders, such as peptic ulcers.
Uses & Indications
Dosage
Adults
For H. pylori eradication, typically Omeprazole 20 mg twice daily + Metronidazole 500 mg twice daily (often with a third antibiotic like Amoxicillin or Clarithromycin) for 7-14 days, as directed by physician.
Elderly
No specific dose adjustment typically required, but monitor for side effects due to potential altered pharmacokinetics.
Renal_impairment
Metronidazole dose reduction may be necessary in severe renal impairment (CrCl <10 mL/min). No omeprazole adjustment for mild to moderate.
How to Take
Take the tablet whole with water, preferably before meals. Do not chew or crush.
Mechanism of Action
Omeprazole, a Proton Pump Inhibitor, reduces stomach acid production by irreversibly blocking the H+/K+-ATPase pump in gastric parietal cells. Metronidazole, a nitroimidazole antibiotic, exerts its bactericidal effect by damaging microbial DNA, particularly against anaerobic bacteria and certain protozoa.
Pharmacokinetics
Onset
Omeprazole: Acid secretion inhibition within 1 hour. Metronidazole: Rapid.
Excretion
Omeprazole: Primarily renal (70-80%). Metronidazole: Primarily renal (60-80%), some fecal.
Half life
Omeprazole: 0.5-1 hour (plasma), longer duration of action due to irreversible pump inhibition. Metronidazole: 6-11 hours.
Absorption
Omeprazole is rapidly absorbed, bioavailability ~30-40%. Metronidazole is rapidly and completely absorbed, bioavailability >90%.
Metabolism
Omeprazole: Extensively metabolized by CYP2C19 and CYP3A4 in the liver. Metronidazole: Hepatic metabolism via oxidation.
Side Effects
Contraindications
- •Hypersensitivity to Omeprazole, Metronidazole, or any component
- •Known neurological disorders (for Metronidazole)
- •First trimester of pregnancy (Metronidazole)
Drug Interactions
Alcohol
Disulfiram-like reaction (Metronidazole)
Warfarin
Increased anticoagulant effect (Metronidazole)
Clopidogrel
Reduced antiplatelet effect (Omeprazole)
Phenytoin/Phenobarbital
Altered metabolism, increased levels of phenytoin (Metronidazole), decreased omeprazole levels (Phenobarbital)
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture.
Overdose
Symptoms of overdose may include nausea, vomiting, ataxia, disorientation, and seizures. Treatment is symptomatic and supportive; no specific antidote.
