Orpra 20 mg Capsule contains Omeprazole, a proton pump inhibitor, used to reduce the amount of acid produced in the stomach. It treats conditions like heartburn, acid reflux, and peptic ulcers.
Uses & Indications
Dosage
Adults
Typically 20-40 mg once daily, depending on the condition. For GERD, 20 mg once daily for 4-8 weeks. For duodenal ulcer, 20 mg once daily for 2-4 weeks. For gastric ulcer, 20 mg once daily for 4-8 weeks.
Elderly
No dosage adjustment is generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment is generally required for patients with impaired renal function.
How to Take
Orally, preferably in the morning before a meal. Swallow the capsule whole with water. Do not chew or crush.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the parietal cells of the stomach, thereby blocking the final step in gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour; maximal acid suppression in 2 hours.
Excretion
Mainly excreted in urine (approximately 80%) and feces (approximately 20%) as metabolites.
Half life
0.5-1 hour (terminal half-life), but the duration of acid suppression is much longer (up to 72 hours) due to irreversible binding.
Absorption
Rapidly absorbed from the gastrointestinal tract, though absorption is variable. Bioavailability is approximately 30-40% after a single dose.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system, particularly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to omeprazole or any component of the formulation
- Concomitant use with nelfinavir

Digoxin
Increased digoxin absorption and levels; monitor.
Warfarin
Increased INR/prothrombin time; monitor closely.
Tacrolimus
Increased tacrolimus levels; monitor.
Clopidogrel
Omeprazole can reduce the antiplatelet effect of clopidogrel. Avoid concomitant use or consider alternative antiplatelet therapy.
Methotrexate
Increased methotrexate levels; adjust dose or monitor.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
