Description
Overview of the medicine
Orthocal-D 500 mg tablet is a combination of Calcium and Vitamin D3, used to supplement daily intake and treat conditions related to calcium and vitamin D deficiencies. It helps maintain healthy bones and teeth.
Uses & Indications
Dosage
Adults
1-2 tablets daily, or as directed by the physician. May be taken with or without food.
Elderly
Same as adult dosage, but caution with pre-existing renal impairment or hypercalcemia.
Renal_impairment
Use with caution. Dosage adjustments may be necessary, and monitoring of serum calcium and phosphate levels is crucial.
How to Take
Swallow the tablet whole with water. It can be taken with or without food, but taking it with meals may enhance absorption.
Mechanism of Action
Calcium is an essential mineral for bone formation, muscle contraction, and nerve function. Vitamin D3 (Cholecalciferol) facilitates calcium absorption from the gut and its deposition in bones, regulating calcium and phosphate homeostasis.
Pharmacokinetics
Onset
Supplementation benefits are gradual, taking weeks to months for significant changes in bone density. Serum calcium levels may increase within hours of intake.
Excretion
Calcium is primarily excreted via feces (unabsorbed) and urine. Vitamin D metabolites are excreted primarily in bile and feces, with some renal excretion.
Half life
Calcium half-life varies, but Vitamin D3 has a long half-life, with metabolites persisting for weeks to months.
Absorption
Calcium is absorbed primarily in the small intestine (duodenum) via active transport and passive diffusion. Vitamin D3 is well absorbed from the small intestine, especially in the presence of fats.
Metabolism
Calcium is not metabolized in the traditional sense, but its levels are homeostatically regulated. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol) and then in the kidneys to 1,25-dihydroxyvitamin D (calcitriol), the active form.
Side Effects
Contraindications
- •Hypercalcemia (high calcium levels in blood)
- •Hypercalciuria (high calcium levels in urine)
- •Severe renal impairment
- •Hypersensitivity to any component
- •Active Vitamin D intoxication
Drug Interactions
Levothyroxine
Calcium may impair absorption. Separate administration by several hours.
Bisphosphonates
Calcium can reduce their absorption. Administer at least 30 minutes apart.
Thiazide diuretics
May increase the risk of hypercalcemia.
Digitalis glycosides
Hypercalcemia can potentiate their effects, increasing risk of arrhythmias.
Tetracyclines & Quinolones
Calcium can reduce the absorption of these antibiotics. Administer at least 2-3 hours apart.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include hypercalcemia (nausea, vomiting, constipation, polyuria, polydipsia, muscle weakness, confusion, fatigue). Severe cases can lead to renal impairment and cardiac arrhythmias. Treatment involves discontinuing the supplement, hydration, and medical management of hypercalcemia.
Pregnancy & Lactation
Generally considered safe and often recommended during pregnancy and lactation to meet increased calcium and vitamin D requirements, but should be used under medical supervision.
