Overview of the medicine
Oscoral-D 500 mg Tablet is a combination of calcium and vitamin D3, used to prevent and treat calcium and vitamin D deficiencies, and to support bone health.
Uses & Indications
Dosage
Adults
Usually 1-2 tablets daily, or as directed by a physician. May be taken in divided doses.
Elderly
Same as adult dosage, but caution should be exercised and calcium levels monitored, especially in those with impaired renal function.
Renal_impairment
Use with caution. Dosage adjustments may be necessary based on creatinine clearance and serum calcium/phosphate levels. Contraindicated in severe renal impairment.
How to Take
Take the tablet orally, preferably with a meal, to enhance absorption. Do not chew or crush the tablet; swallow it whole with water.
Mechanism of Action
Calcium Carbonate provides essential calcium ions for bone mineralization, muscle contraction, and nerve function. Vitamin D3 (Cholecalciferol) is crucial for regulating calcium and phosphate homeostasis, facilitating their absorption from the gut, and promoting bone formation and remodeling.

Onset
Therapeutic effects on bone mineral density are gradual, taking weeks to months.
Excretion
Unabsorbed calcium is excreted in feces. Absorbed calcium is mainly excreted via kidneys (urine) and intestines (feces). Vitamin D metabolites are primarily excreted in bile/feces and some in urine.
Half life
The half-life of circulating Vitamin D3 is approximately 24 hours, but its active metabolites have varying half-lives (e.g., calcitriol 3-6 hours). Calcium's half-life is variable depending on physiological state.
Absorption
Calcium Carbonate is absorbed from the gastrointestinal tract, primarily in the duodenum and proximal jejunum, requiring stomach acid. Vitamin D3 is well absorbed from the gastrointestinal tract in the presence of bile salts.
Metabolism
Vitamin D3 is hydroxylated in the liver to 25-hydroxycholecalciferol (calcidiol) and then in the kidneys to the active form, 1,25-dihydroxycholecalciferol (calcitriol). Calcium is not metabolized.
Side Effects
Contraindications
- •Hypersensitivity to the active substances or to any of the excipients.
- •Hypercalcemia (abnormally high levels of calcium in the blood).
- •Hypercalciuria (abnormally high levels of calcium in the urine).
- •Severe renal impairment (e.g., dialysis patients).
- •Nephrolithiasis (kidney stones) or nephrocalcinosis.
- •Primary hyperparathyroidism.
Drug Interactions
Digoxin
Hypercalcemia can potentiate the effects of digoxin, increasing the risk of cardiac arrhythmias.
Bisphosphonates
Calcium can interfere with the absorption of bisphosphonates. Administer calcium at least 30 minutes after or 2 hours before bisphosphonates.
Corticosteroids
Can reduce calcium absorption and increase calcium excretion.
Thiazide diuretics
May reduce urinary excretion of calcium, increasing the risk of hypercalcemia.
Tetracycline and Quinolone antibiotics
Calcium can form insoluble complexes, reducing their absorption. Administer at least 2-3 hours before or 4-6 hours after calcium supplements.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia with symptoms like nausea, vomiting, constipation, abdominal pain, muscle weakness, polyuria, polydipsia, and in severe cases, cardiac arrhythmias, kidney damage, and coma. Treatment involves discontinuing the medication, hydration, and medical management of hypercalcemia.
Pregnancy & Lactation
This medication is generally considered safe and often recommended during pregnancy and lactation for adequate calcium and vitamin D intake. However, it should be used under medical supervision, and daily intake of calcium should not exceed 2500 mg and vitamin D 4000 IU to avoid hypercalcemia in the mother and fetus.
