Description
Overview of the medicine
Omeprazole is a proton pump inhibitor used to treat conditions caused by excess stomach acid, such as ulcers, GERD, and erosive esophagitis.
Uses & Indications
Dosage
Adults
Typically 20 mg once daily for 4-8 weeks for GERD and ulcers. Higher doses may be used for severe conditions like Zollinger-Ellison syndrome.
Elderly
No specific dose adjustment is generally required, but use with caution due to potential decreased liver function.
Renal_impairment
No dose adjustment is needed for patients with impaired renal function.
How to Take
Take orally, preferably in the morning, at least 30 minutes before a meal. Swallow the capsule whole with water; do not chew or crush it.
Mechanism of Action
Omeprazole works by irreversibly blocking the H+/K+-ATPase enzyme system (proton pump) in the gastric parietal cells, thereby reducing gastric acid secretion.
Pharmacokinetics
Onset
1-2 hours
Excretion
Mainly via urine (approximately 77%) and feces (approximately 18%).
Half life
0.5-1 hour (terminal elimination half-life is longer due to irreversible binding to the proton pump)
Absorption
Rapidly absorbed from the gastrointestinal tract after oral administration. Bioavailability is approximately 30-40%, increasing with repeated dosing.
Metabolism
Extensively metabolized in the liver via cytochrome P450 enzymes (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR and bleeding risk.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel (clinical significance debated, often avoided).
Methotrexate
Increased methotrexate levels, particularly with high-dose therapy.
Diazepam, Phenytoin
Increased plasma levels of these drugs.
Nelfinavir, Atazanavir
Reduced plasma concentrations and efficacy of antivirals.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include confusion, drowsiness, blurred vision, tachycardia, nausea, vomiting, sweating, and headache. Management is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and potential benefits outweigh risks. Omeprazole is excreted in breast milk; use with caution in lactating women.

- Monitor for signs of Vitamin B12 deficiency with prolonged use (>3 years).
Doctor Notes
- Advise patients about potential long-term risks of PPIs, especially with prolonged use.
- Consider using the lowest effective dose for the shortest possible duration to manage symptoms.
- Evaluate symptoms suggestive of underlying serious conditions (e.g., malignancy) before initiating PPI therapy.
