Mechanism of Action
Calcium is essential for bone formation, muscle contraction, and nerve function. Vitamin D3 aids in calcium absorption from the gut and its deposition in bones. Magnesium is involved in bone mineralization and over 300 enzymatic reactions. Zinc is crucial for bone formation, immune function, and wound healing.
Pharmacokinetics
Onset
Nutritional effects are gradual and sustained over time with regular use.
Excretion
Calcium: Primarily excreted via feces, some via urine. Vitamin D3: Excreted via bile and feces. Magnesium & Zinc: Primarily excreted via kidneys and feces.
Half life
Calcium: Variable (bone turnover). Vitamin D3: 15-30 days for calcifediol (25-OH D3), several hours for calcitriol (1,25-(OH)2 D3). Magnesium & Zinc: Variable, depend on body stores and intake.
Absorption
Calcium: Primarily absorbed in the small intestine, enhanced by Vitamin D. Vitamin D3: Absorbed in the small intestine, requires bile salts. Magnesium & Zinc: Absorbed primarily in the small intestine.
Metabolism
Calcium: Not metabolized, distributed. Vitamin D3: Metabolized in the liver to 25-hydroxyvitamin D3, then in the kidneys to active 1,25-dihydroxyvitamin D3. Magnesium & Zinc: Primarily distributed and utilized in various tissues.
Side Effects
Contraindications
- •Hypercalcemia (high levels of calcium in the blood)
- •Hypervitaminosis D (excess vitamin D in the body)
- •Severe renal impairment
- •Known hypersensitivity to any of the components
Drug Interactions
Digoxin
High calcium levels can increase the risk of digoxin toxicity.
Levothyroxine
Calcium can reduce absorption. Take calcium at least 4 hours after levothyroxine.
Bisphosphonates
Calcium can interfere with bisphosphonate absorption. Take calcium at least 30 minutes after bisphosphonates (or as advised for specific bisphosphonate).
Thiazide diuretics
May decrease urinary calcium excretion, increasing risk of hypercalcemia.
Tetracycline antibiotics
Calcium can reduce absorption of tetracyclines. Take calcium at least 2 hours before or 4-6 hours after tetracyclines.
Fluoroquinolone antibiotics
Calcium can reduce absorption of fluoroquinolones. Take calcium at least 2 hours before or 4-6 hours after fluoroquinolones.
Antacids containing aluminum or magnesium
Long-term use may affect calcium and phosphate balance.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include hypercalcemia (nausea, vomiting, constipation, confusion, polydipsia, polyuria) or hypervitaminosis D. Treatment involves discontinuing the supplement, hydrating the patient, and managing symptoms. Seek immediate medical attention.
Pregnancy & Lactation
