Description
Overview of the medicine
Ostocurb 500 mg Tablet is a calcium supplement used to prevent or treat low blood calcium levels in people who do not get enough calcium from their diets. It is crucial for healthy bones, muscles, nervous system, and heart function.
Uses & Indications
Dosage
Adults
Typically 500 mg to 1500 mg of elemental calcium daily, in divided doses, preferably with meals. For osteoporosis prevention: 1000-1200 mg daily.
Elderly
Similar to adults, but often higher daily requirements due to decreased absorption. Up to 1200-1500 mg elemental calcium daily.
Renal_impairment
Use with caution. Dosage may need adjustment based on serum calcium and phosphate levels. Often used as a phosphate binder in specific doses under medical supervision.
How to Take
Take orally, preferably with meals to enhance absorption. Tablets can be swallowed whole or chewed.
Mechanism of Action
Calcium carbonate works by providing elemental calcium, which is an essential mineral required for maintaining bone density, nerve impulse transmission, muscle contraction, and blood coagulation. It directly replenishes the body's calcium stores.
Pharmacokinetics
Onset
Therapeutic effects on bone density are gradual, over weeks to months. Antacid effects are rapid.
Excretion
Mainly excreted via feces (unabsorbed calcium) and urine (excess absorbed calcium). Renal excretion is regulated to maintain calcium homeostasis.
Half life
The half-life of calcium in the body is complex due to its constant turnover and regulation by parathyroid hormone and calcitonin.
Absorption
Calcium carbonate absorption is variable, generally around 20-30%, and is enhanced by Vitamin D. Absorption is best when taken with food.
Metabolism
Elemental calcium is not metabolized in the traditional sense; it is an ion that participates in physiological processes.
Side Effects
Contraindications
- •Hypercalcemia (high blood calcium levels)
- •Severe renal impairment
- •Hypersensitivity to calcium carbonate or any component of the formulation
- •Hypercalciuria (high calcium levels in urine)
Drug Interactions
Bisphosphonates
Calcium can interfere with the absorption of bisphosphonates. Administer separately by at least 30 minutes to 2 hours.
Iron supplements
Calcium may reduce iron absorption. Administer separately.
Thiazide diuretics
Can increase serum calcium levels, potentially leading to hypercalcemia.
Digitalis glycosides
Hypercalcemia caused by calcium supplements can increase the risk of digitalis toxicity.
Thyroid hormones (Levothyroxine)
Calcium can decrease levothyroxine absorption. Separate by at least 4 hours.
Tetracycline and Fluoroquinolone antibiotics
Calcium can decrease the absorption of these antibiotics. Separate administration by at least 2-4 hours.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose (hypercalcemia) include nausea, vomiting, constipation, abdominal pain, muscle weakness, confusion, excessive thirst, and frequent urination. In severe cases, it can lead to kidney damage and cardiac arrhythmias. Treatment involves discontinuing calcium, hydration, and medical management of hypercalcemia.
Pregnancy & Lactation
Calcium is essential during pregnancy and lactation. It is generally considered safe when taken at recommended doses. Pregnant and breastfeeding women should consult their doctor for appropriate dosage.
