Description
Overview of the medicine
Otecon (Omeprazole) is a proton pump inhibitor (PPI) used to reduce stomach acid production. It is prescribed for conditions like heartburn, acid reflux, peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD: 20 mg once daily for 4-8 weeks. For Duodenal Ulcer: 20 mg once daily for 2-4 weeks. For Gastric Ulcer: 20 mg once daily for 4-8 weeks. For Zollinger-Ellison Syndrome: Initial dose 60 mg once daily, adjusted as per patient need.
Elderly
No specific dose adjustment is required for the elderly, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is generally required for patients with renal impairment.
How to Take
Take Otecon (Omeprazole) capsules whole with a glass of water, usually in the morning before a meal. Do not chew or crush the capsules.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase enzyme (the proton pump) in the gastric parietal cells. This action blocks the final step in acid production, leading to a significant and prolonged reduction in gastric acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression occurs within 1 hour.
Excretion
Approximately 77% of a dose is excreted renally, and the remainder is excreted in the feces.
Half life
Plasma half-life is 0.5-1 hour, but the duration of acid suppression is much longer.
Absorption
Rapidly absorbed from the gastrointestinal tract. Bioavailability is approximately 30-40% due to first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 enzyme system, specifically CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Omeprazole or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
May increase INR and risk of bleeding.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel.
Methotrexate
May increase methotrexate levels and toxicity.
Nelfinavir, Atazanavir
May significantly decrease the plasma levels of these antiretrovirals.
Diazepam, Phenytoin, Citalopram
May increase plasma levels of these drugs due to inhibition of hepatic metabolism.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, tachycardia, nausea, vomiting, dizziness, abdominal pain, and headache. Treatment is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
Pregnancy: Category C (older classification), studies suggest no increased risk of major congenital malformations. Use only if clearly needed. Lactation: Omeprazole is excreted in breast milk. Use with caution during breastfeeding.
