Otegal is a broad-spectrum antibiotic combination containing Ofloxacin, a fluoroquinolone, and Ornidazole, a nitroimidazole. It is used to treat various bacterial and protozoal infections.
Uses & Indications
Dosage
Adults
Usually one Otegal tablet (Ofloxacin 200 mg + Ornidazole 500 mg) orally twice daily for 5-7 days, depending on the infection severity.
Elderly
No specific dose adjustment is usually required for elderly patients unless there is significant renal impairment.
Renal_impairment
Dose adjustment is necessary for patients with impaired renal function. Consult physician for specific guidelines.
How to Take
To be taken orally, preferably with or after meals to minimize gastrointestinal discomfort.
Mechanism of Action
Ofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential for DNA replication, transcription, repair, and recombination. Ornidazole is converted to reactive metabolites that damage microbial DNA, leading to cell death.

Onset
Therapeutic concentrations are generally achieved within 1-2 hours for Ofloxacin and similar for Ornidazole.
Excretion
Ofloxacin is primarily excreted unchanged via the kidneys. Ornidazole is excreted via both renal and fecal routes.

Ofloxacin has a half-life of 4-6 hours, while Ornidazole has a longer half-life of 12-14 hours.

Both Ofloxacin and Ornidazole are rapidly and well absorbed from the gastrointestinal tract after oral administration.

Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, requiring INR monitoring.
Theophylline
Increased plasma levels of Theophylline, risk of toxicity.
Cimetidine, Phenytoin, Barbiturates
May affect the metabolism of Ornidazole.
Antacids containing magnesium/aluminum
May decrease the absorption of Ofloxacin. Administer at least 2 hours apart.
