Description
Overview of the medicine
Oticlor 500 mg Capsule contains Cefaclor, a second-generation cephalosporin antibiotic used to treat various bacterial infections including respiratory tract infections, skin and soft tissue infections, urinary tract infections, and otitis media.
Uses & Indications
Dosage
Adults
Typical adult dosage is 250 mg to 500 mg every 8 hours. Max 4 g/day.
Elderly
Dosage adjustment not generally necessary unless severe renal impairment is present.
Renal_impairment
For severe renal impairment (creatinine clearance < 10 mL/min), initial dose is as usual, subsequent doses may be reduced or frequency extended.
How to Take
Administer orally. May be taken with or without food. Taking with food may minimize gastrointestinal upset.
Mechanism of Action
Cefaclor is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to osmotic instability and cell lysis.
Pharmacokinetics
Onset
Clinical effects typically observed within hours to days, depending on the infection.
Excretion
Mainly via the kidneys by glomerular filtration and tubular secretion. Approximately 60-85% of the dose is excreted unchanged in urine within 8 hours.
Half life
Approximately 0.5-1 hour in individuals with normal renal function.
Absorption
Well absorbed from the gastrointestinal tract (approximately 60-85%). Peak plasma concentrations are reached within 0.5-1 hour after administration.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cefaclor or other cephalosporins
- History of severe hypersensitivity reaction to penicillin (due to potential cross-reactivity)
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin. Monitor INR/PT.
Probenecid
Increases cefaclor plasma levels and prolongs its half-life by inhibiting renal excretion.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, and diarrhea. Management is primarily supportive; activated charcoal may be given. Hemodialysis is generally not effective.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy; benefits should outweigh risks. Cefaclor is excreted in breast milk in small amounts; use with caution during lactation.
