Description
Overview of the medicine
Ovaxa is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, swelling, and stiffness caused by various conditions such as osteoarthritis and rheumatoid arthritis. It works by reducing substances in the body that cause pain and inflammation.
Uses & Indications
Dosage
Adults
For osteoarthritis: 100 mg once or twice daily. For rheumatoid arthritis: 100 mg once or twice daily. For acute pain and primary dysmenorrhea: 200 mg initial dose, followed by 100 mg every 12 hours as needed. Maximum 400 mg/day.
Elderly
No specific dose adjustment is generally required, but caution is advised. Initiate with the lowest effective dose and monitor renal function.
Renal_impairment
Use with caution in mild to moderate renal impairment. Not recommended in severe renal impairment (CrCl < 30 mL/min).
How to Take
Ovaxa 100 mg Capsule should be taken orally, with or without food. However, taking it with food may help minimize gastrointestinal upset. Swallow the capsule whole with a glass of water; do not crush, chew, or open it.
Mechanism of Action
Ovaxacib selectively inhibits the cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins involved in inflammation, pain, and fever. By inhibiting COX-2, it reduces the production of these pro-inflammatory mediators.
Pharmacokinetics
Onset
30-60 minutes for analgesic effect.
Excretion
Excreted mainly via urine (approx. 50-60%) and feces (approx. 25-30%) as inactive metabolites.
Half life
Approximately 8-12 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentration is reached within 2-3 hours after oral administration.
Metabolism
Primarily metabolized in the liver by the cytochrome P450 CYP2C9 enzyme.
Side Effects
Contraindications
- Hypersensitivity to Ovaxacib or any component of the formulation.
- History of asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs.
- Severe heart failure (NYHA Class II-IV).
- Active gastrointestinal bleeding or peptic ulcer.
- Severe renal impairment (CrCl < 30 mL/min) or severe hepatic impairment.
Drug Interactions
Lithium
May increase plasma lithium levels, leading to toxicity, by reducing renal clearance of lithium.
Methotrexate
May increase methotrexate toxicity by reducing its renal clearance.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Fluconazole (CYP2C9 inhibitor)
May increase Ovaxacib plasma concentrations, requiring dose reduction.
Warfarin and other anticoagulants
Increased risk of bleeding due to interference with platelet function and potential for displacement from plasma protein binding sites.
ACE Inhibitors, Angiotensin II Receptor Blockers (ARBs), and Diuretics
May reduce the antihypertensive effect of these medications and increase the risk of renal impairment, especially in dehydrated or elderly patients.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of NSAID overdose typically include lethargy, drowsiness, nausea, vomiting, epigastric pain, which are generally reversible with supportive care. Gastrointestinal bleeding can occur. Rarely, hypertension, acute renal failure, respiratory depression, and coma may occur. Management involves symptomatic and supportive treatment. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C (first and second trimesters); Category D (third trimester). Ovaxacib should be avoided during late pregnancy (third trimester) due to potential for premature closure of the fetal ductus arteriosus and potential renal dysfunction. Use during the first and second trimesters only if the potential benefit justifies the potential risk to the fetus. It is not known whether Ovaxacib is excreted in human milk; use with caution in lactating women, and consult a doctor.

Clinical Trials
Information on clinical trials for Ovaxacib is available through publicly accessible clinical trial registries and manufacturer's reports. Studies typically focus on efficacy in pain and inflammation, and safety profile in various patient populations.
Lab Monitoring
- Periodically monitor renal function (e.g., serum creatinine, BUN) in patients receiving long-term therapy, especially in the elderly or those with pre-existing renal impairment.
- Monitor liver function tests (e.g., ALT, AST) if signs or symptoms of liver dysfunction occur.
- Monitor blood pressure regularly.
- For patients on concurrent anticoagulant therapy, monitor INR/PT levels frequently.
Doctor Notes
- Counsel patients on the black box warnings regarding cardiovascular and gastrointestinal risks, particularly for those with pre-existing conditions or risk factors.
- Monitor renal function periodically, especially in elderly patients, those with heart failure, or concurrent diuretic therapy.
- Consider the lowest effective dose for the shortest duration consistent with individual patient treatment goals.
- Assess concomitant medications for potential drug interactions, especially with anticoagulants, lithium, ACE inhibitors, and diuretics.
Patient Guidelines
- Take Ovaxa exactly as prescribed by your doctor. Do not exceed the recommended dose or duration of treatment.
- Swallow the capsule whole; do not crush, chew, or open it.
- Inform your doctor about all other medications you are taking, especially blood thinners, diuretics, or other NSAIDs.
- Report any unusual bleeding, persistent stomach pain, dark stools, or swelling immediately to your doctor.
Missed Dose Advice
If you miss a dose of Ovaxa, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Ovaxa may cause dizziness, drowsiness, or visual disturbances in some patients. If you experience these effects, avoid driving or operating heavy machinery.
Lifestyle Advice
- Avoid smoking, as it can increase the risk of cardiovascular and gastrointestinal side effects.
- Limit or avoid alcohol consumption while taking Ovaxa, as it may increase the risk of stomach bleeding.
- Maintain a healthy diet and stay hydrated.
