Description
Overview of the medicine
Oxaprozin is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, swelling, and stiffness caused by conditions such as osteoarthritis and rheumatoid arthritis. It works by blocking certain natural substances in the body that cause inflammation.
Uses & Indications
Dosage
Adults
Initial dose: 5 mg once daily, adjusted based on patient response and tolerability, up to a maximum of 10-20 mg per day in divided doses.
Elderly
Start with the lowest effective dose (e.g., 5 mg once daily) and monitor closely for adverse effects. Dose adjustment may be necessary due to decreased renal function.
Renal_impairment
Use with caution. Dosage reduction (e.g., 5 mg every other day) may be required depending on the degree of impairment. Not recommended for severe renal impairment.
How to Take
Take orally, preferably with food or milk to minimize gastrointestinal upset. Swallow the tablet whole with a glass of water.
Mechanism of Action
Oxaprozin inhibits prostaglandin synthesis by reversibly inhibiting cyclooxygenase (COX-1 and COX-2) enzymes. This leads to a reduction in inflammation, pain, and fever.
Pharmacokinetics
Onset
Analgesic effect: 1-2 hours; Anti-inflammatory effect: several days to 2 weeks for full therapeutic benefit.
Excretion
Mainly excreted by the kidneys (approximately 60% as metabolites, 5% as unchanged drug) and about 35% in feces.
Half life
Approximately 42-58 hours, allowing for once-daily dosing.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations are reached in approximately 3-6 hours after oral administration.
Metabolism
Extensively metabolized in the liver, primarily through oxidation and glucuronidation to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to oxaprozin or other NSAIDs
- Active peptic ulcer or gastrointestinal bleeding
- Severe renal or hepatic impairment
- Third trimester of pregnancy
- Coronary Artery Bypass Graft (CABG) surgery
Drug Interactions
Lithium
May increase plasma lithium levels, leading to toxicity.
Warfarin
Increased risk of bleeding due to enhanced anticoagulant effect.
Methotrexate
May increase plasma methotrexate levels and toxicity.
ACE inhibitors/Diuretics
May reduce the antihypertensive effect and impair renal function.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include lethargy, drowsiness, nausea, vomiting, epigastric pain. Management is primarily supportive. Gastric decontamination (e.g., activated charcoal) may be considered if ingestion is recent. Monitor renal function and electrolyte balance.
Pregnancy & Lactation
Pregnancy Category C (D in third trimester). Avoid use in the third trimester due to potential premature closure of the fetal ductus arteriosus. Use during lactation should be avoided or used with caution, as NSAIDs can be excreted in breast milk.
