Omeprazole is a proton pump inhibitor used to reduce stomach acid production and treat conditions like GERD, ulcers, and heartburn.
Uses & Indications
Dosage
Adults
Typical dose is 20 mg orally once daily, preferably in the morning before food. Duration varies based on condition, often 4-8 weeks.
Elderly
No dose adjustment is generally needed, but caution should be exercised in severe hepatic impairment.
Renal_impairment
No dose adjustment is required for patients with renal impairment.
How to Take
Take orally, preferably in the morning before food. Swallow the capsule whole with water; do not chew, crush, or open the capsule.
Mechanism of Action
Irreversibly blocks the H+/K+-ATPase proton pump in gastric parietal cells, thereby reducing gastric acid secretion.
Pharmacokinetics
Onset
Acid secretion inhibition begins within 1 hour, with maximal effect seen within 4 days of therapy.
Excretion
Mainly excreted via urine (approx. 80%) and feces (approx. 20%) as metabolites.
Half life
Plasma half-life is approximately 0.5-1 hour.
Absorption
Rapidly absorbed from the gastrointestinal tract. Bioavailability is approximately 30-40%.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased systemic exposure to digoxin.
Diazepam
Delayed elimination of diazepam.
Warfarin
Increased INR and prothrombin time, increasing risk of bleeding.
Clopidogrel
Reduced antiplatelet effect, potentially increasing risk of cardiovascular events.
Methotrexate
Increased and prolonged serum levels of methotrexate.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, dizziness, abdominal pain, and headache. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks. Exercise caution during lactation, as omeprazole is excreted in breast milk.
