Description
Overview of the medicine
p-cool-15-10-cream is a topical analgesic cream containing methyl salicylate and menthol. It is used to relieve pain in muscles, joints, and tendons.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected area 3-4 times daily, or as directed by a physician.
Elderly
No specific dose adjustment required, apply as for adults.
Renal_impairment
Use with caution in severe renal impairment due to systemic absorption of salicylates.
How to Take
For external use only. Wash hands before and after application. Do not apply to open wounds, damaged skin, or mucous membranes. Avoid contact with eyes.
Mechanism of Action
Methyl Salicylate is a topical NSAID that inhibits prostaglandin synthesis, providing anti-inflammatory and analgesic effects. Menthol acts as a counterirritant, producing a cooling sensation that distracts from pain.
Pharmacokinetics
Onset
Within minutes for menthol's cooling effect, 15-30 minutes for analgesic effects.
Excretion
Metabolites of both are primarily excreted in urine.
Half life
Salicylic acid (from methyl salicylate) has a half-life of 2-4 hours, menthol's is shorter.
Absorption
Both methyl salicylate and menthol are absorbed percutaneously. Absorption rate can vary with skin integrity, area of application, and occlusion.
Metabolism
Methyl salicylate is hydrolyzed to salicylic acid primarily in the liver. Menthol is metabolized in the liver via glucuronidation.
Side Effects
Contraindications
- Hypersensitivity to salicylates (e.g., aspirin) or any component of the cream
- Children under 2 years of age
- Application to broken or irritated skin
- Asthma or nasal polyps (due to salicylate cross-reactivity)
Drug Interactions
Other NSAIDs
Concurrent use of other NSAIDs, especially systemic ones, may increase the risk of adverse effects.
Oral Anticoagulants (e.g., Warfarin)
Topical salicylates can be absorbed systemically and may potentiate the effects of oral anticoagulants, increasing bleeding risk.
Storage
Store below 30°C in a dry place, away from direct sunlight. Keep out of reach of children.
Overdose
Systemic toxicity from topical overdose is rare but possible with large area application or damaged skin. Symptoms include nausea, vomiting, tinnitus, dizziness, and hyperventilation. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Use with caution during pregnancy, especially in the third trimester, due to potential salicylate effects on the fetus (e.g., premature ductus arteriosus closure). Consult a physician. Limited data on excretion into breast milk; use with caution during lactation.
