Description
Overview of the medicine
P-Proton 20 mg tablet is a proton pump inhibitor used to reduce stomach acid production, primarily for conditions like GERD, ulcers, and acid reflux.
Uses & Indications
Dosage
Adults
Typically 20 mg once daily, taken before breakfast. Dosage may be increased to 40 mg daily depending on the condition.
Elderly
No specific dose adjustment is required for elderly patients, but caution is advised in those with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
How to Take
Take the tablet whole with a glass of water, preferably in the morning before breakfast. Do not crush, chew, or break the tablet.
Mechanism of Action
P-Proton works by irreversibly binding to and inhibiting the H+/K+-ATPase enzyme (the proton pump) in the gastric parietal cells, thereby blocking the final step of acid secretion.
Pharmacokinetics
Onset
Within 2-3 hours, full effect within 2-4 days of continuous treatment.
Excretion
Primarily excreted in urine (approx. 70-80%) and feces (approx. 20%) as metabolites.
Half life
Approximately 1 hour (terminal half-life), but acid suppression effect is longer due to irreversible binding.
Absorption
Rapidly absorbed from the gastrointestinal tract; peak plasma concentrations reached within 2-4 hours.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, particularly CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to p-proton, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir (an antiretroviral drug).
Drug Interactions
Clopidogrel
May reduce the antiplatelet effect of clopidogrel.
Methotrexate
May increase methotrexate levels, especially with high-dose methotrexate.
Iron Salts, Vitamin B12
May reduce the absorption of drugs that depend on gastric pH for absorption, like iron salts and vitamin B12.
Ketoconazole, Itraconazole
Absorption may be decreased due to reduced gastric acidity.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.

There is limited experience with overdose. Management should be symptomatic and supportive. P-Proton is not removed by hemodialysis.
