Description
Overview of the medicine
Pace 500 mg Tablet contains Paracetamol, a widely used analgesic and antipyretic medicine. It is primarily used to relieve mild to moderate pain and reduce fever. It works by blocking the production of certain chemical messengers that cause pain and fever in the brain.
Uses & Indications
Dosage
Adults
500 mg to 1000 mg every 4-6 hours as needed. Maximum 4000 mg in 24 hours.
Elderly
Usually same as adult dosage, but caution with liver/renal impairment.
Renal_impairment
Dosage adjustment may be necessary in severe renal impairment (e.g., 500 mg every 6-8 hours).
How to Take
Oral administration. May be taken with or without food. Swallow the tablet whole with water.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system by inhibiting prostaglandin synthesis, particularly COX-2. It also acts on hypothalamic heat-regulating centers to produce antipyresis. Its peripheral anti-inflammatory activity is minimal.

Excretion
Excreted primarily in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
1.25 to 3 hours
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 10-60 minutes after oral administration.
Metabolism
Mainly metabolized in the liver by conjugation with glucuronic acid and sulfate. A small portion is metabolized by cytochrome P450 to a highly reactive intermediate (NAPQI), which is normally detoxified by conjugation with glutathione.

Drug Interactions
Alcohol
Increases risk of hepatotoxicity.
Warfarin
May increase anticoagulant effect, leading to bleeding risk.
Cholestyramine
Reduces absorption of paracetamol.
Carbamazepine, Phenobarbital, Phenytoin
May increase risk of hepatotoxicity.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, abdominal pain, and liver damage. Treatment involves gastric lavage, activated charcoal, and administration of N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Considered safe for use during pregnancy and lactation when used at recommended doses, but always consult a doctor.
