Description
Overview of the medicine
Paclitex 6 mg Injection contains Paclitaxel, an antineoplastic agent used in the treatment of various cancers. It works by interfering with cell division.
Uses & Indications
Dosage
Adults
Dosage varies widely based on indication, combination therapy, and patient's physical condition. Typical doses range from 80-260 mg/m² administered intravenously over 1 or 3 hours every 3 weeks.
Elderly
No specific dose adjustment purely based on age, but close monitoring for adverse effects is recommended.
Renal_impairment
No specific dosage recommendations for mild to moderate renal impairment. For severe impairment, caution and dose reduction may be necessary.
How to Take
Administered intravenously, usually as a 1-hour or 3-hour infusion. Pre-medication with corticosteroids, antihistamines, and H2 antagonists is required to prevent hypersensitivity reactions.
Mechanism of Action
Paclitaxel promotes the assembly of microtubules from tubulin dimers and stabilizes them, preventing depolymerization. This inhibits normal dynamic reorganization of the microtubule network essential for vital interphase and mitotic cellular functions.
Pharmacokinetics
Onset
Effects are observed during the mitotic phase of the cell cycle.
Excretion
Mainly biliary excretion (via feces), with a small amount excreted renally.
Half life
Initial half-life: 0.1-1.5 hours; terminal half-life: 3-52.7 hours (dose-dependent).
Absorption
Administered intravenously, absorption is complete.
Metabolism
Primarily hepatic, via CYP2C8 and CYP3A4 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to Paclitaxel or Cremophor EL (polyoxyethylated castor oil)
- •Baseline neutrophil count < 1,500 cells/mm³ for solid tumors or < 1,000 cells/mm³ for Kaposi's sarcoma
- •Severe hepatic impairment
Drug Interactions
Cisplatin
Increased myelosuppression when Paclitaxel is given after Cisplatin.
CYP2C8/CYP3A4 inducers (e.g., Rifampin, Phenytoin)
May decrease Paclitaxel levels and efficacy.
CYP2C8/CYP3A4 inhibitors (e.g., Ketoconazole, Erythromycin)
May increase Paclitaxel levels and toxicity.
Storage
Store unopened vials at 20°C to 25°C (68°F to 77°F). Protect from light. Do not freeze.
Overdose
Overdose symptoms include severe myelosuppression, peripheral neurotoxicity, and mucositis. Management is supportive, including blood transfusions, granulocyte colony-stimulating factors, and antiemetics.
Pregnancy & Lactation
Contraindicated during pregnancy due to potential fetal harm (Category D). Avoid breastfeeding due to excretion into breast milk and potential harm to the infant.
