Painac-TR 100 mg Capsule contains Tramadol Hydrochloride, an opioid analgesic used to relieve moderate to severe pain. It is formulated as an extended-release capsule to provide long-lasting pain relief.
Uses & Indications
Dosage
Adults
Initial dose 100 mg once daily. May be titrated up by 100 mg every 5 days, not exceeding 300 mg daily.
Elderly
For patients >75 years, maximum 300 mg daily. Dosage adjustment may be needed based on renal/hepatic function.
Renal_impairment
Creatinine clearance <30 mL/min: Not recommended for extended-release formulations. For immediate-release, dose reduction and increased dosing interval are needed.
How to Take
Swallow the capsule whole with water. Do not chew, crush, or break the capsule. Can be taken with or without food.
Mechanism of Action
Tramadol acts as a centrally acting opioid analgesic. It has a dual mechanism of action: it binds to μ-opioid receptors and inhibits the reuptake of norepinephrine and serotonin, thereby enhancing inhibitory pain pathways.
Pharmacokinetics
Onset
Typically within 1 hour for immediate release; extended-release forms provide sustained action.
Excretion
Primarily excreted renally (about 90%) as metabolites and unchanged drug.
Half life
Approximately 6-7 hours (parent drug); active metabolite (M1) half-life is slightly longer.
Absorption
Rapidly and almost completely absorbed after oral administration. Bioavailability of extended-release formulations is high but may vary.
Metabolism
Extensively metabolized in the liver by O-demethylation (via CYP2D6) and N-demethylation (via CYP3A4, CYP2B6), followed by conjugation of the O-demethylated metabolite with glucuronic acid.
Side Effects
Contraindications
- •Acute intoxication with alcohol, hypnotics, analgesics, opioids, or other psychotropic drugs.
- •Patients receiving MAO inhibitors or within 14 days of discontinuing them.
- •Severe respiratory depression.
- •Uncontrolled epilepsy.
- •Breastfeeding mothers (for extended-release formulations due to risk to infant).
Drug Interactions
Quinidine
Increased tramadol exposure and risk of side effects.
Carbamazepine
Decreased tramadol efficacy due to increased metabolism.
MAO inhibitors
Increased risk of serotonin syndrome and seizures.
Serotonergic drugs (e.g., SSRIs, SNRIs, TCAs)
Increased risk of serotonin syndrome.
CNS depressants (e.g., benzodiazepines, alcohol)
Additive CNS depression, respiratory depression, hypotension, profound sedation, coma, death.
Storage
Store at room temperature (below 30°C) in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms include respiratory depression, somnolence, coma, seizures, skeletal muscle flaccidity, cold and clammy skin, and pinpoint pupils. Treatment involves maintaining a patent airway, providing ventilatory support, and administering naloxone to reverse opioid effects. Gastric lavage may be considered.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during pregnancy, especially near term, due to risk of neonatal opioid withdrawal syndrome. Excreted in breast milk; breastfeeding is not recommended due to potential infant adverse effects.
