Description
Overview of the medicine
Paloxiron is a highly potent selective serotonin 5-HT3 receptor antagonist used for the prevention and treatment of chemotherapy-induced nausea and vomiting (CINV) and postoperative nausea and vomiting (PONV).
Uses & Indications
Dosage
Adults
0.075 mg intravenous injection administered once prior to chemotherapy.
Elderly
No specific dose adjustment required based on age.
Renal_impairment
No dose adjustment required for mild to moderate renal impairment. Data limited for severe impairment.
How to Take
Administer intravenously over 30 seconds as a single dose. Should be given prior to the start of chemotherapy.
Mechanism of Action
Blocks serotonin 5-HT3 receptors in the central nervous system and the gastrointestinal tract, preventing their activation by serotonin, which is released during chemotherapy and causes nausea and vomiting.
Pharmacokinetics
Onset
Within 30 minutes.
Excretion
Mainly renal excretion of metabolites, with a small portion excreted unchanged in urine and feces.
Half life
Approximately 40 hours.
Absorption
Rapidly absorbed after intravenous injection.
Metabolism
Primarily hepatic metabolism via CYP enzymes, with minor contributions from CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to paloxiron or any component of the injection.
- Patients with congenital long QT syndrome.
Drug Interactions
Dexamethasone
May enhance the antiemetic effect of paloxiron.
CYP3A4 inducers/inhibitors
Caution with strong inducers (e.g., rifampicin) or inhibitors (e.g., ketoconazole) due to potential alteration in metabolism, though unlikely to be clinically significant given the drug's primary metabolism pathway.
Storage
Store at 20°C to 25°C (68°F to 77°F); excursions permitted to 15°C to 30°C (59°F to 86°F). Protect from light. Do not freeze.
Overdose
No specific antidote. Treatment should be symptomatic and supportive. ECG monitoring is recommended due to potential for QT prolongation.
Pregnancy & Lactation
Use only if clearly needed and the potential benefits outweigh the potential risks to the fetus/infant. Consult a healthcare professional.

Lab Monitoring
- ECG monitoring in patients at risk for QT prolongation.
- Baseline and periodic electrolyte monitoring.
Doctor Notes
- Remind patients to report any cardiac symptoms, especially if they have pre-existing cardiac conditions.
- Administer slowly to minimize risk of hypotension or bradycardia.
Patient Guidelines
- Report any unusual symptoms or adverse reactions to your doctor immediately.
- Ensure you inform your doctor about all other medications you are taking.
- Do not self-administer; it is an injection for professional use only.
Missed Dose Advice
As it is a single-dose injection administered by a healthcare professional, a missed dose is unlikely. If part of a multi-day regimen (unlikely for 0.075 mg single dose), consult your doctor immediately.
Driving Precautions
May cause dizziness or fatigue. Advise caution when driving or operating machinery until individual response is known.
Lifestyle Advice
- Maintain adequate hydration.
