Overview of the medicine
Panfast 40 mg Injection contains Pantoprazole, a proton pump inhibitor used to reduce stomach acid production. It is prescribed for conditions like gastroesophageal reflux disease (GERD), stomach ulcers, and Zollinger-Ellison syndrome, especially when oral administration is not possible.
Uses & Indications
Dosage
Adults
Typically 40 mg once daily by intravenous infusion over 2-15 minutes, or as a continuous infusion. Duration of IV therapy should be as short as possible, usually 7-10 days, then switch to oral therapy.
Elderly
No dosage adjustment is generally necessary, but caution is advised due to potential for reduced hepatic function.
Renal_impairment
No dosage adjustment is required for patients with renal impairment or undergoing hemodialysis.
How to Take
Administer intravenously as a slow infusion over 2 to 15 minutes, or as a continuous infusion. The solution should be prepared by dissolving the powder in the provided solvent and further diluting if necessary.
Mechanism of Action
Pantoprazole irreversibly binds to and inhibits the H+/K+-ATPase (proton pump) in the secretory canaliculi of gastric parietal cells, thereby suppressing the final step in gastric acid secretion.
Pharmacokinetics
Onset
Acid suppression begins within 15-30 minutes.
Excretion
Mainly excreted renally (approx. 80%) and the remainder in feces via biliary excretion.
Half life
Approximately 1 hour (terminal half-life), but the duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapidly absorbed after intravenous administration. Bioavailability is 100% via IV route.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 (CYP) enzymes, mainly CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation.
- Patients with severe hepatic impairment for long-term use (consult physician).
Drug Interactions
Warfarin
Increased INR and prothrombin time have been reported, requiring monitoring.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel by inhibiting its conversion to active metabolite. Concomitant use should be approached with caution.
Methotrexate
May increase and prolong serum levels of methotrexate and/or its metabolite, particularly with high doses of methotrexate. Consider temporary withdrawal of PPI.
Atazanavir, Nelfinavir
Concomitant use with PPIs is not recommended due to significantly reduced plasma concentrations of antiretroviral drugs, potentially leading to loss of therapeutic effect and development of resistance.
Storage
Store below 30°C in a dry place, away from light. Do not freeze. Reconstituted solution is stable for a limited time (e.g., 24 hours at room temperature).
Overdose
In case of overdose, seek immediate medical attention. Management is symptomatic and supportive. Pantoprazole is not removed by hemodialysis.

Pregnancy Category C. Use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Pantoprazole is excreted into breast milk; caution is advised when administered to a nursing mother.
