Description
Overview of the medicine
This medicine is used for the relief of mild to moderate pain and fever. It contains Paracetamol, an analgesic and antipyretic, combined with Caffeine, which enhances the pain-relieving effect, especially for headaches.
Uses & Indications
Dosage
Adults
1-2 tablets (Paracetamol 500-1000 mg, Caffeine 65-130 mg) every 4-6 hours as needed. Do not exceed 8 tablets (4 grams Paracetamol) in 24 hours.
Elderly
Same as adults, but caution should be exercised in individuals with impaired liver or kidney function.
Renal_impairment
For severe renal impairment (creatinine clearance < 10 ml/min), the dosing interval for paracetamol should be extended to 8 hours. Consult a doctor.
How to Take
Take orally with a glass of water. Can be taken with or without food.
Mechanism of Action
Paracetamol acts primarily in the central nervous system, inhibiting prostaglandin synthesis. Caffeine is a central nervous system stimulant that enhances the analgesic effect of paracetamol and has mild vasoconstrictive properties.
Pharmacokinetics
Onset
Analgesic effect begins within 30-60 minutes.
Excretion
Mainly excreted in the urine as metabolites.
Half life
Paracetamol: 1 to 4 hours; Caffeine: 3 to 7 hours.
Absorption
Both paracetamol and caffeine are rapidly and almost completely absorbed from the gastrointestinal tract.
Metabolism
Paracetamol is primarily metabolized in the liver via glucuronidation and sulfation. Caffeine is extensively metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to paracetamol, caffeine, or any other component of the formulation.
- •Severe hepatic impairment.
Drug Interactions
Alcohol
Increased risk of hepatotoxicity with chronic alcohol consumption.
Mexiletine, Cimetidine
May decrease caffeine metabolism, leading to increased caffeine levels.
Warfarin and other coumarins
May increase the anticoagulant effect and risk of bleeding with prolonged regular daily use of paracetamol.
Other paracetamol-containing products
Increased risk of paracetamol overdose.
Certain antiepileptics (e.g., carbamazepine, phenytoin)
May increase the risk of paracetamol hepatotoxicity.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose of paracetamol can cause severe, sometimes fatal, liver damage. Symptoms include nausea, vomiting, abdominal pain, and anorexia. Caffeine overdose can cause restlessness, nervousness, insomnia, tremors, and tachycardia. Treatment for paracetamol overdose involves N-acetylcysteine. Supportive care for caffeine overdose.
Pregnancy & Lactation
