Overview of the medicine
Pantoprazole is a proton pump inhibitor (PPI) that decreases the amount of acid produced in the stomach.
Uses & Indications
Dosage
Adults
40 mg once daily for 4-8 weeks for GERD. May increase to 40 mg twice daily for erosive esophagitis.
Elderly
Generally, no dosage adjustment is needed, but consider lower doses due to potential for decreased renal function.
Renal_impairment
No dosage adjustment is generally required in patients with renal impairment.
How to Take
Take orally 30-60 minutes before a meal. For injection, administer intravenously.
Mechanism of Action
Pantoprazole inhibits the H+/K+-ATPase enzyme system (the proton pump) of the gastric parietal cell, blocking the final step in acid production.

Onset
Within 2.5 hours.
Excretion
Renal (80%) and fecal (20%).
Half life
Approximately 1 hour.
Absorption
Rapidly absorbed after oral administration; bioavailability ~77%.
Metabolism
Hepatic via CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to pantoprazole or any of the excipients
- •Concurrent use with rilpivirine
Drug Interactions
Warfarin
May increase INR and prothrombin time. Monitor INR closely.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel.
Storage
Store at room temperature, away from moisture and heat.
Overdose
Limited data available. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when pantoprazole is administered to a nursing woman.
