Description
Overview of the medicine
Pantofast 20 mg Tablet contains Pantoprazole, a proton pump inhibitor, used to treat conditions where the stomach produces too much acid, such as heartburn, acid reflux, ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Typically 20-40 mg once daily, taken before a meal. For GERD/Erosive Esophagitis: 40 mg once daily for up to 8 weeks. For Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted based on response.
Elderly
No specific dose adjustment is generally required, but caution is advised in those with severe hepatic impairment.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment or those on hemodialysis.
How to Take
Take the tablet whole, do not chew or crush. It is usually taken once daily in the morning, before breakfast.
Mechanism of Action
Pantoprazole is a selective proton pump inhibitor that irreversibly blocks the H+/K+-ATPase enzyme system (proton pump) in the gastric parietal cells, thereby inhibiting the final step in gastric acid secretion.

Onset
Acid suppression typically begins within 2.5 hours.
Excretion
Primarily excreted in the urine (approx. 71%) and feces (approx. 18%) as metabolites.
Half life
Approximately 1 hour (terminal half-life), but acid suppression lasts much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration, bioavailability is approximately 77%.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications

Drug Interactions
Warfarin
Increased INR and prothrombin time.
Methotrexate
Increased methotrexate levels, especially with high doses.
Iron salts, Ketoconazole, Itraconazole
Reduced absorption due to increased gastric pH.
HIV Protease Inhibitors (e.g., Atazanavir)
Reduced absorption and efficacy.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and include lethargy, confusion, or mild gastrointestinal upset. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use during pregnancy only if clearly needed. Excreted in human milk, exercise caution during breastfeeding.
