Overview of the medicine
Parix (Paroxetine) is an antidepressant medication primarily used to treat major depressive disorder, obsessive-compulsive disorder, panic disorder, social anxiety disorder, generalized anxiety disorder, and post-traumatic stress disorder.
Uses & Indications
Dosage
Adults
Initial dose 20 mg once daily, usually in the morning. May be increased by 10 mg increments at weekly intervals to a maximum of 50 mg/day for most indications.
Elderly
Initial dose 10 mg once daily. Max dose typically 40 mg/day.
Renal_impairment
For severe renal impairment (creatinine clearance <30 mL/min), initial dose 10 mg once daily, max dose 40 mg/day. Similar dose reduction for moderate to severe hepatic impairment.
How to Take
Parix should be taken orally, preferably with food, usually once daily in the morning to minimize the risk of insomnia.
Mechanism of Action
Paroxetine selectively inhibits the reuptake of serotonin (5-HT) into presynaptic neurons in the central nervous system, thereby potentiating serotonergic activity in the brain.
Pharmacokinetics
Onset
Antidepressant effects usually observed after 2-4 weeks of treatment.
Excretion
Excreted in urine (approximately 64%) and feces (approximately 36%).
Half life
Approximately 21 hours (range 3-65 hours).
Absorption
Well absorbed from the gastrointestinal tract; peak plasma concentrations reached in about 3-8 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 enzyme.
Side Effects
Contraindications
- •Hypersensitivity to paroxetine or any component of the formulation.
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI treatment.
- •Concomitant use with Thioridazine or Pimozide.
Drug Interactions
MAOIs
Risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability, mental status changes).
Triptans
Increased risk of serotonin syndrome.
Warfarin
Increased risk of bleeding due to interference with platelet aggregation.
Tamoxifen
Reduced efficacy of tamoxifen due to CYP2D6 inhibition.
NSAIDs, Aspirin
Increased risk of gastrointestinal bleeding.
Tricyclic Antidepressants (TCAs)
Increased plasma concentrations of TCAs, increasing risk of side effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, nausea, vomiting, tremor, tachycardia, and serotonin syndrome. Management involves supportive care, maintaining airway, and monitoring vital signs. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category D: Paroxetine may cause fetal harm, including cardiac malformations and persistent pulmonary hypertension in neonates. Use only if potential benefit outweighs risk. Excreted in breast milk, use with caution during lactation.
