Overview of the medicine
Parlox (Paroxetine) is a Selective Serotonin Reuptake Inhibitor (SSRI) antidepressant used to treat major depressive disorder, obsessive-compulsive disorder (OCD), panic disorder, social anxiety disorder, and post-traumatic stress disorder (PTSD).
Uses & Indications
Dosage
Adults
Initial dose of 20 mg once daily, usually in the morning with food. May be increased by 10 mg increments at weekly intervals to a maximum of 50 mg/day for depression and 60 mg/day for OCD/panic disorder.
Elderly
Initial dose of 10 mg once daily. Max dose typically 40 mg/day.
Renal_impairment
For severe renal impairment (creatinine clearance < 30 mL/min), initial dose 10 mg/day, max 40 mg/day.
How to Take
Take orally, once daily, usually in the morning with food to minimize gastrointestinal upset. Do not chew or crush tablets.
Mechanism of Action
Paroxetine selectively inhibits the reuptake of serotonin (5-HT) into presynaptic neurons in the central nervous system, thereby enhancing serotonergic neurotransmission.

Onset
Clinical effects may take 2-4 weeks to become noticeable.
Excretion
Eliminated primarily via urine (64%) and feces (36%) as metabolites.
Half life
Approximately 21 hours (range 10-24 hours).
Absorption
Well absorbed after oral administration, peak plasma concentrations typically achieved within 3-8 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6.
Side Effects
Contraindications
- •Hypersensitivity to paroxetine or any component of the formulation.
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI treatment.
- •Concomitant use with Thioridazine or Pimozide.
Drug Interactions
MAOIs
Risk of serotonin syndrome, may be fatal.
Alcohol
Avoid concomitant use due to enhanced CNS depressant effects.
Warfarin, NSAIDs
Increased risk of bleeding.
Digoxin, Theophylline
Paroxetine may increase plasma levels of these drugs.
Triptans, Tramadol, Linezolid
Increased risk of serotonin syndrome.
Cimetidine, Phenobarbital, Phenytoin
May alter paroxetine plasma levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, dizziness, somnolence, sinus tachycardia, and dilated pupils. Serotonin syndrome may occur. Treatment is supportive, focusing on maintaining an airway and monitoring cardiac and vital signs.
Pregnancy & Lactation
Pregnancy Category D. Paroxetine can cause fetal harm when administered to a pregnant woman. It is also excreted in breast milk. Use only if potential benefit justifies the potential risk to the fetus/infant. Consult a doctor.
