Description
Overview of the medicine
Pasmodil (Hyoscine Butylbromide) is an antispasmodic medication used to relieve cramps or spasms of the stomach, intestines, and bladder, often associated with conditions like irritable bowel syndrome (IBS), renal or biliary colic.
Uses & Indications
Dosage
Adults
10-20 mg orally, 3-5 times daily. Maximum 100 mg/day.
Elderly
No specific dosage adjustment usually required, but caution is advised.
Renal_impairment
No specific dosage adjustment required for mild to moderate impairment. Use with caution in severe renal impairment.
How to Take
Oral tablets can be taken with or without food. Injections are administered intravenously, intramuscularly, or subcutaneously by a healthcare professional.
Mechanism of Action
Hyoscine Butylbromide is a quaternary ammonium derivative of hyoscine, which means it does not readily cross the blood-brain barrier. It acts as a competitive antagonist at muscarinic acetylcholine receptors, primarily in the smooth muscles of the gastrointestinal tract, biliary tract, and genitourinary tract, thereby producing a spasmolytic effect.
Pharmacokinetics
Onset
30-60 minutes (oral), 15 minutes (parenteral).
Excretion
Primarily excreted via faeces (about 50%) and urine (about 30%) as unchanged drug and metabolites.
Half life
Approximately 5 hours (for systemic elimination).
Absorption
Poorly absorbed from the gastrointestinal tract (less than 8%).
Metabolism
Mainly metabolized in the liver (hydrolysis).
Side Effects
Contraindications
- Myasthenia gravis
- Megacolon
- Narrow-angle glaucoma
- Prostatic hypertrophy with urinary retention
- Mechanical stenosis of the gastrointestinal tract
- Paralytic or obstructive ileus
- Tachycardia
Drug Interactions
Antihistamines
May increase antimuscarinic effects.
Beta-adrenergic agents
May increase heart rate.
Tricyclic antidepressants
May potentiate antimuscarinic effects.
Dopamine antagonists (e.g., Metoclopramide)
May reduce effects of both drugs on gastrointestinal motility.
Storage
Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include dry mouth, blurred vision, tachycardia, urinary retention. Treatment is symptomatic and supportive, including physostigmine if severe anticholinergic symptoms are present.
Pregnancy & Lactation
Use with caution during pregnancy if clearly needed, as insufficient data exists. Excretion into breast milk is unlikely due to poor absorption, but use with caution during lactation.
