Description
Overview of the medicine
Pazopanib is a multi-targeted receptor tyrosine kinase inhibitor used for the treatment of advanced renal cell carcinoma (RCC) and advanced soft tissue sarcoma (STS). It targets vascular endothelial growth factor receptor (VEGFR)-1, -2, and -3, platelet-derived growth factor receptor (PDGFR)-α and -β, and c-Kit, inhibiting angiogenesis and tumor cell proliferation.
Uses & Indications
Dosage
Adults
800 mg once daily, taken at least 1 hour before or 2 hours after a meal.
Elderly
No specific dose adjustment required based on age, but monitor for adverse effects.
Renal_impairment
No dose adjustment required for mild to moderate renal impairment. Data limited for severe impairment.
How to Take
Oral, once daily, without food. Swallow tablets whole, do not crush or chew.
Mechanism of Action
Pazopanib inhibits multiple receptor tyrosine kinases (RTKs) including VEGFR-1, -2, -3, PDGFR-α, -β, and c-Kit. By inhibiting these kinases, it blocks cell proliferation, angiogenesis, and tumor survival.
Pharmacokinetics
Onset
Not precisely defined, anti-tumor effects are gradual.
Excretion
Primarily via feces (63%), with a minor amount excreted in urine (<4%).
Half life
Approximately 30.6 hours.
Absorption
Approximately 21% oral bioavailability; Tmax is 2-4 hours; absorption increases with food.
Metabolism
Extensively metabolized by CYP3A4, with minor contributions from CYP1A2 and CYP2C8.
Side Effects
Contraindications
- •Severe hepatic impairment
- •Hypersensitivity to pazopanib or any excipients
Drug Interactions
QT-prolonging agents
Increased risk of QT prolongation, monitor ECG.
Strong CYP3A4 inducers (e.g., rifampin, phenytoin)
Decrease pazopanib exposure, avoid co-administration.
Strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin)
Increase pazopanib exposure, reduce pazopanib dose.
Storage
Store at room temperature (20-25°C), away from moisture and light. Keep out of reach of children.
Overdose
No specific antidote. Management of overdose should involve general supportive measures. Monitor for signs of toxicity, especially hepatotoxicity and cardiac effects.
Pregnancy & Lactation
Pregnancy Category D. May cause fetal harm. Advise females of reproductive potential to use effective contraception. Not recommended during breastfeeding; discontinue breastfeeding.
